摘  要:目的探讨对老年原发性高血压(EH)患者血清超敏C-反应蛋白(hs-CRP)、单核细胞趋化蛋白-1(MCP-1)与可溶性细胞间粘附分子-1(sICAM-1)与动态脉压(PP)的相关性。方法选取174例老年EH患者,采用动态血压监测仪测量受试者全天平均收缩压、全天平均舒张压、并计算动态脉压值,按照动态脉压大小(<60或≥60 mm Hg)分为两组,用免疫比浊法和酶联免疫吸附测定法测定两组患者血清hs-CRP、MCP-1与sICAM-1水平,用Spearman相关和多元线性回归分析动态脉压与三者的相关性。结果与PP<60 mm Hg组相比;PP≥60 mm Hg组的hs-CRP、sICAM-1水平较高[分别(4.86±6.92)比(2.33±2.75)mg/L,(2885.71±604.42)比(2525.75±560.02)ng/L;均P<0.05,两组MCP-1水平差异无统计学意义(P>0.05);Spearman相关分析表明hs-CRP(r=0.251、P<0.01)、sICAM-1(r=0.294、P<0.01)均与动态脉压呈正相关。多元线性逐步回归分析示:在排除其他危险因素影响后,sICAM-1、MCP-1、hs-CRP与动态脉压仍相关。结论 hs-CRP、MCP-1与sICAM-1参与了老年EH患者动态脉压升高的病理生理学过程。Objective To investigate the relationship between serum high-sensitivity C-reactive protein (hs- CRP), monocyte chemoattractant protein-1 ( MCP-1 ), soluble intercellular adhesion molecule-1 ( sICAM-1 ) and pulse pressure (PP) in the old patients with essential hypertension (EH). Methods A total of 174 old patients with EH were recruited. Their systolic blood pressure, diastolic blood pressure were measured by ambulatory blood pressure mo- nitoring to calculate PP. The patients were divided into two groups by PP 〈 60 or ≥60 mm Hg. The levels of serum hs- CRP,MCP-1 and sICAM-1 were detected by immune turbidimetry and the enzyme-linked immunosorbent assay. Spearman correlation analysis and multiple linear regression was used to analyze the relationship between serum hs- CRP,MCP-1 ,slCAM-1 and PP. Results The levels of serum hs-CRP and slCAM-1 were higher in PP≥60 mm Hg group than those in PP 〈60 mm Hg group(4.86 ±6.92 vs. 2.33±2.75mg/L,2885.71 ±604.42 vs. 2525.75 ± 560.02 ng/L,respectively ;all P 〈0.05 ). There was no significant difference on the level of MCP-1 between the two groups (P 〉 0, 05 ). Spearman correlation analysis showed serum hs-CRP (r = 0.251, P 〈 0.01 ), sICAM-1 (r = 0.294, P 〈 0.01 )were positively related with PP. Multiple linear stepwise regression analysis showed serum slCAM-1, MCP-1 and hs-CRP were correlated with PP after excluding other factors. Conclusion hs-CRP, MCP-land slCAM-1 paly a important role in the pathophysiologieal process of the increasing of PP in old patients with hypertension.
