影响高龄经产妇妊娠及分娩的相关因素分析及应对措施探讨

摘　　要：目的分析影响高龄经产妇妊娠结局及分娩方式的因素,了解年龄与妊娠结局、分娩方式等因素之间的关系,探讨应对措施。方法将2010年1月至2011年7月分娩的高龄经产妇390例作为观察组,将同期非高龄经产妇513例作为对照组,对分娩方式、妊娠并发症、新生儿情况等进行比较分析。结果 (1)观察组妊娠并发症发生率为53.1%,对照组为31.0%,两组比较,差异有统计学意义(P<0.05)。(2)观察组新生儿窒息率及死亡率明显高于对照组,差异有统计学意义(P<0.01)。(3)两组剖宫产率比较,差异无统计学意义(P>0.05)。结论高龄经产妇妊娠风险高,新生儿结局不良,应加强孕期健康宣教,做好妊娠期及分娩期监护,加强新生儿护理,促进母儿康复。Objective To analyze the factors influencing the pregnancy outcome and the delivery mode in elderly multi- paras, to know the relationship between age with the pregnancy outcome and the delivery mode and to explore the countermea- sures. Methods 390 elderly multiparas with delivery from January 2010 to July 2011 were regarded as the observation group and at the same period 513 non-elderly multiparas as the control group. The delivery modes, pregnant complications and comorbidities and neonatal condition were compared between the two groups and analyzed. Results （ 1 ） The occurrence rate of pregnant com- plications and comorbidities was 53.1% in the observation group and 31.0% in the control group,difference between two groups showing statistical significance （P〈0.05）. （2）The neonatal asphyxia rate and the mortality rate in the observation group were obvi- ously higher than those in the control group with statistical difference between the two groups （P〈0.05）. （3）There was no statisti- cal difference in the cesarean section rates between the two groups（P〉0.05 ）. Conclusion Elderly muhiparas have a high risk in pregnancy and poor neonatal outcome. So it is necessary to strengthen the pregnant health education, take better monitoring in pregnant and delivery periods, strengthen neonatal nursing care in order to promote maternal and neonatal rehabilitation.
