分类层递护理对抑郁障碍患者康复效果

摘　　要：目的比较分类层递护理和常规护理对抑郁障碍患者的康复效果。方法制定一套适用于抑郁障碍患者的分类层递护理操作手册。将64例符合标准的患者分为分类层递护理组(简称干预组)和常规护理组(简称对照组)各32例,治疗6周,用汉密顿抑郁量表(HAMD)评定两组治疗前和治疗后第2,4,6周末的减分值、6周后疗效、6个月随访时的复发/再燃率,并评定各期依从性。结果①第2周末两组有明显差异,第6周末差异极显著,第2周末对照组和干预组HAMD值分别为(16.54±3.951)和(14.60±3.286),第6周末两组HAMD值分别为(9.31±2.768)和(8.23±2.333);②第6周末两组疗效差异有统计学意义(P<0.01),其中对照组痊愈率为37.5%,显效率为87.5%,干预组痊愈率为43.7%,显效率为93.8%;③随访6个月时,干预组有1例患者症状复发,复发率3.1%,而对照组7例症状复发(其中2例再燃),复发率21.9%。结论分类层递护理对抑郁障碍患者的康复效果显著。Objective To compare the classification layer delivery nursing and conventional nursing on patients with depressive disorder rehabilitation effect.Methods Developing a set of applicable to patients with depressive disorder classification layer delivery nursing operation manual.A total of 64 patients were divided into classification layer delivery care group（the treatment group）and routine nursing group（the control group）,each of 32 cases,after treatment with 6 weeks,Hamilton Depression Rating Scale was used to assess in two groups before treatment and after treatment 2,4,6 weekend minus score,6 weeks after treatment,6 months follow-up at the time of relapse or combustion rate,and assess the compliance.Results ①Two groups had significant difference after 2 weekend,the extremely significant difference after 6 weekend,2 weekend in two groups the HAMD values were（16.44±3.951）and（14.60±3.286）,6 weekend HAMD（9.31±2.768）and（8.33±2.333）;②There were statistically differences between two groups after 6 weekend（P0.01）,which controls the cure rate was 37.5%,the effective rate was 87.5%,the treatment group cure rate was 43.7%,the effective rate was 93.8%.③Follow-up 6 months,1 patients had symptoms relapse in psychological group,recurrence rate were 3.1%,while the control group of 7cases of recurrence of symptoms（including 2 cases of reburning）,recurrence rate were 21.9%.Conclusion The effect is remarkable of classification layer delivery nursing on depression rehabilitation.
