摘  要:目的观察预吸入沙丁胺醇对不同推注速度下舒芬太尼引发咳嗽反应的影响,以探讨更为平稳的全身麻醉的诱导用药方法。方法选择160例择期行全身麻醉下手术患者,年龄25-62岁,ASAⅠ-Ⅱ级,随机分为Ⅰ-Ⅳ组,每组40人。所有患者无麻醉前用药,Ⅰ、Ⅲ组(实验组)患者预吸入沙丁胺醇一揿200μg,Ⅱ、Ⅳ组(对照组)则吸入雾化生理盐水。10分钟后各组进行全麻诱导,其中Ⅰ、Ⅱ组为快推注组,静脉推注舒芬太尼0.5μg/kg,推注时间3s;Ⅲ、Ⅳ组为慢推注组,静脉推注舒芬太尼0.5μg/kg,推注时间为30s。观察和记录推注舒芬太尼后2min内咳嗽的发生情况。结果Ⅰ、Ⅳ组咳嗽发生率比Ⅱ组低(P<0.05),Ⅲ组咳嗽发生率明显比I组低(P<0.01),差异有统计学意义。结论推注速度可影响舒芬太尼引起的咳嗽反应的发生,而预吸入沙丁胺醇可有效降低不同推注速度舒芬太尼引发咳嗽的发生率,尤以慢推注组效果明显,是使麻醉诱导更为平稳的较好方法。Objective To evaluate the effect of pre-inhalation of salbutamol on cough reflex induced by different in jection speed of sufentanyl to investigate a more stable in du cti0n therapy method ofgeneral anesthesia. Methods 160 patients Aged Z5-62yr,ASAI- Ⅱ , undergoing elective surgery were randomied into four groups of 40 each . Group Ⅱ. IV served as control received saline aerosol,while Group I . Ⅲ received an pre-inhalation of one metered, aerosol puff of salbultamol(200 μg) 10min prior to intravenous injection ofsufentanyl. Sufentanyl(0.5μg/kg) was administered over 3 s in Group I .Ⅱ , whereas for patients in GroupⅢ , Ⅳ the sufentanyl was given over a period of 30 s. The concentration of sufentanyl was 10μg/ml. To observe and record the Incidence of cough induced by sufentanyl during 2 minitues after injection. Results The incidence of cough in group I . IV was less than that in group Ⅱ(P〈0.05). The incidence of cough in GroupⅢ was signnificantly less than that in groupⅡ. (P〈0.01)Conclusion The injection speed can influence the cough induced by sufentanyl,and the pre inhalation of salbutamol can effectively reduce the incidence of cough in- duced by sufentanyl in different injection rate,espcially in slow injection speed. The pre-inhalation of salbutamol along with slow injection is a better method which makes the anesthesia induction more steady.
