保留与不保留股骨颈型人工全髋关节置换术30例疗效分析

摘　　要：目的：比较保留与不保留股骨颈型人工全髋关节置换的治疗效果。方法：2006年9月-2011年3月，我科应用保留股骨颈型人工全髋关节置换术治疗髋关节骨关节病10例，不保留股骨颈型人工全髋关节置换术治疗髋关节骨关节病20例。比较两种方式的手术时间，术中出血，住院时间，人工关节活动范围等疗效指标和术后关节脱住，假体松等并发症。结果：两组在手术时间，术中出血，住院时间，人工关节活动范围方面无明显统计学差异，两组经过0．5～5年随访，30个人工髋关节的活动和其后关节功能良好，X线摄片示人工髋关节位置良好，假体无松动和下沉。结论：保留与不保留股骨颈型人工全髋关节置换术在治疗效果上无明显差异，但保留股骨颈型人工全髋关节置换符合股骨近端生理顺应性，使假体适应应力分布。防止股骨近端骨质疏松引起假体松动和下沉。同时为今后需要假体翻修提供骨结构基础。故在非老年人骨质疏松股骨颈骨折患者中应尽量使用保留股骨颈型人工全髋关节置术。Objective.. To compare the therapeutic efficacy between with or without reservation the neck of femur dur-ing total hip replacement. Methods： During September of 2006 to March of 2011,We had operated 10 patients with while 20 patients without reservation the neck of femur during total hip replacement. We compared the herapeutic effect such as operation time,bleeding, length of stay,range of motion of artificial joint and so on,and the complication such as dearticu-lation, prosthesis cinch and so on. Results ： Two groups had no obvious statistics difference between operation time, bleed-ing, length of stay, range of motion of artificial joint. Two groups were followed up for half to five years, thirty artificial hip joint had good activity and joint function, the X ray showed artificial hip joint had well position,there were no prosthe-sis cinch and subsidence observed. Conclusions ： There was no difference between the therapeutic efficacy with or without reservation the neck of femur during total hip replacement. But There were some superiorities for reservation the neck of femur during total hip replacement, because it fit well with the femur proximal end physiological complaisance. It make the prosthesis fit the stress. It prevented prosthesis cinch and subsidence from the femur proximal end osteoporosis. It pro-vided bone architecture foundation for revision. So we should operate with patients not so old with reservation the neck of femur during total hip replacement as far as possible.
