摘  要:目的探讨经尿道前列腺电切术治疗超高龄良性前列腺增生的安全性和有效性。方法应用经尿道前列腺电切术治疗超高龄(80岁以上)前列腺增生患者134例,随访6-60个月,对安全性和有效性相关指标进行统计分析。结果切除前列腺组织质量18-90 g,平均(42±20)g;手术时间40-150 min,平均(94±51)min;失血量70-420 ml,平均(240±164)ml;术后住院时间5-10 d,平均8.3 d;无电切综合征发生。随访6-60个月,平均37个月。IPSS评分从术前26.5±5.0降至9.0±4.5;生存质量评分从术前5.1±0.9降至2.4±0.6;最大尿流率从术前0-8 ml/s(平均2.7 ml/s)增至9-22 ml/s(平均14 ml/s);残余尿量从术前65-500 ml(平均176 ml)降至20-70 ml(平均52 ml)。上述指标术前、术后对比均有显著性差异(P<0.05)。结论经尿道前列腺电切术是治疗超高龄良性前列腺增生安全且有效的手术方式。Objective To investigate the safety and effectiveness of transurethral resection of prostate(TURP) for benign prostatic hyperplasia(BPH) in super-aged patients.Methods TURP was applied to 134 super-aged(≥80 years old) and high risk patients with BPH.Patients were followed up for 6-60 months,and the preoperative and postoperative indices were compared.Results The mean weight of resected prostatic tissue was(42±20) g(18-90 g),the operative time of 134 cases averaged(94±51)min(40-150 min),blood loss was(240±164)ml(70-420 ml),the postoperative hospital stay averaged 8.3 d(5-10 d),and no transurethral resection syndrome(TURS) occurred.The average follow-up time was 37 months.International prostate symptom score(IPSS) decreased from 26.5±5.0 to 9.0±4.5,the score of quality of life(QOL) decreased from 5.1±0.9 to 2.4±0.6,the maximun flow rate(Qmax) increased from 2.7 ml/s(0-8 ml/s) to 14 ml/s(9-22 ml/s),and the residual urine volume(RUV) decreased form 176 ml(65-500 ml) to 52ml(20-70 ml).There was significant difference in all the indices before and after surgery(P0.05).Conclusion TURP is safe and effective in the treatment of BPH in super-aged patients.
