卵圆钳免气囊分离器无钉合在完全腹膜外腹腔镜疝修补术中的应用

摘　　要：目的探讨采用卵圆钳免气囊分离器无钉合行完全腹膜外腹腔镜疝修补术(TEP)的可行性和安全性。方法选自2007-05～2011-12在硬膜外麻醉下采用卵圆钳免气囊分离器无钉合行TEP共72例(76侧),其中腹股沟斜疝62例,腹股沟直疝10例。术中不固定补片。结果手术均获得成功,手术时间单侧(70.8±23.8)min,双侧(115.0±20.6)min,术中出血量(10.3±4.6)ml,术后住院时间(3.5±1.2)d,恢复日常活动时间(5.2±1.3)d,术后阴囊积液或血肿10例次(13.2%),阴囊积气6例次(7.9%),经过多次穿刺抽吸后均痊愈。随访12～18个月,未见复发或腹股沟区疼痛等并发症。结论采用卵圆钳免气囊分离器无钉合行TEP是一种可行、安全、有效的无张力疝修补技术,术后疼痛轻、恢复快、复发率低,值得在基层医院推广应用。Objective To explore the feasibility and safety of application balloon dissector-free, unstapled mesh, improved totally extraperitoneal laparoscopic hernia repair（TEP） by ring forceps. Methods From May 2007 to December 2011,72 patients with inguinal hernia（76 sides） received above operation under epidural anesthesia. Among them, there were 62 patients with indirect inguinal hernia, 10 patients with direct inguinal hernia. The patch was not fixed during operation. Results The operations were successful in all patients. The operation time of single side was（70. 8± 23.8 ） rain, the double side（ 115.0 ± 20.6 ） rain ; the amount of bleeding during operation was （ 10.3 ± 4. 6）ml, the postoperative hospitalized time was（3. 5 ± 1.2）d, the resumed daily activity time was（5.2 ± 1.3）d,the postoperative scrotal hydrocele or hematoma was found in 10 case-times（ 13.2% ） ,the scrotum pneumatosis in 6 casetimes（7.9% ）. These diseases can be cured by repeated puncture and aspiration. No other operation complications was found. Dttring follow up of 12 months to 18 months, there was no recurrence and groin pain and other complications. Conclusion The balloon dissector-free, unstapled mesh TEP is a feasible, safe and effective tension-free hernioplasty, and has advantages of less pain, quicker recovery and low recurrence rate.
