摘  要:目的观察腰椎间盘突出组织中的Dickkopf-1(DKK-1)、白细胞介素20(IL-20)和肿瘤坏死因子α(TNF-α)水平变化,探讨其临床意义。方法采用酶联免疫吸附法(ELISA)法测定73例腰椎间盘突出组织标本的DKK-1、IL-20和TNF-α水平,并与15例正常椎间盘组织进行对比,分析其与Pfirriman分级标准的关系。结果观察组DKK-1、IL-20和TNF-α水平分别为(3835.52±419.48)pg/ml、(60.17±8.62)pg/ml和(7.41±2.02)pg/ml,对照组分别为(1524.26±241.93)pg/ml、(15.53±4.17)pg/ml和(2.38±0.79)pg/ml,观察组明显高于对照组,差异均有统计学意义(t分别=3.27、2.53、2.39,P均<0.05),椎间盘突出症患者各亚组退变椎间盘组织DKK-1水平随着疾病的病变程度增加而升高(F=2.76,P<0.05),DKK-1水平与Pfirriman分级呈正相关(r=0.56,P<0.05)。结论腰椎间盘突出组织中的DKK-1、IL-20和TNF-α水平升高。DKK-1可能在椎间盘退行性变中起着重要作用。Objective To observe the level of Dickkopf-1 (DKK-1), interleukin-20(IL-20) and tumor necrosis factor- α(TNF-α) in prolapse of lumbar intervertebral disc degenerative tissue. Methods The levels of DKK-1, IL-20 and TNF-α of 73 cases of prolapse of lumbar intervertebral disc degenerative tissue and 15 cases of normal disc tissue were detected by ELISA and compared. The relationship between the levels of DKK-1, IL-20, TNF-α and Pfirriman grading were analyzed. Results The levels of DKK-1, IL-20 and TNF-α in the observation group were (3835.52±419.48)pg/ml, (60.17±8.62)pg/ml and (7.41±2.02)pg/ml while in the control group were (1524.26±241.93)pg/ml, (15.53±4.17)pg/ml and(2.38±0.79)pg/ml. The differences were statistically significant (t=3.27, 2.53, 2.39, P〈0.05). The level of DKK-1 was positive related to Pfirriman grading (r=0.56, P〈0.05). Conclusions The levels of DKK-1, IL-20 and TNF-α increased in prolapse of lumbar intervertebral disc degenerative tissue. DKK-1 probably play an important role in disc degeneration.
