利多卡因对支气管镜检查的麻醉效果评价

摘　　要：目的评价利多卡因在支气管镜检查中的应用价值。方法选择2011年7月至2012年7月在我院行环甲膜穿刺局部麻醉作支气管镜检查的患者52例,其中男性36例,女性16例。随机分为1%利多卡因浓度组(A组)26例(男性18例,女性8例),2%利多卡因浓度组(B组)26例(男性18例,女性8例)。记录两组患者年龄、利多卡因总量、基础血氧饱和度和最低血氧饱和度、支气管镜操作时间、内镜操作医师视觉评分、严重程度评分及麻醉效果。结果两组患者的年龄、血氧饱和度、操作时间、视觉评分及严重程度评分均无统计学差异,但A组利多卡因应用总量较B组明显要少(76.92±20.35 mg和108.94±38.49 mg,P=0.001)。两组女性患者中利多卡因总量无明显统计学差异,两组不同浓度的利多卡因对支气管检查麻醉效果评定比较无统计学差异。结论 1%浓度的利多卡因在支气管检查过程中具备与2%浓度的利多卡因类似的局部麻醉效果,且应用总剂量也明显减少。Objective To evaluate the clinical value of 1% concentration of lidocaine in bronchoseopy. Methods 52 cases （36 males and 16 females） of patients undergoing bronchoscopy with local anesthesia of cricothyroid membrane puncture were enrolled from July 2011 to July 2012 in our hospital, which consists of 26 objects with 1% concentration of lidocaine（ A group） and 26 with 2% concentration of lidocaine （ B group）. The total doses of lidocaine, basic oxygen saturation, lowest blood oxygen saturation, operation time of bronchoscopy, effect visual score of endoscopic operation, severity score and assessment of anesthesia were recorded. Results There were no significantly statistical difference between two groups for age, blood oxygen saturation, operation time, visual score and severity score. The total doses of lidocaine in A group was obviously less than B group （76.92 ±20.35 mg and 108.94 ±38.49 mg, respectively, P =0.001 ）, but there was no statistical difference in the two groups in female patients. However, there was also no significantly statistical difference between two groups for assessment of anesthesia effect of lidocaine with different concentrations in bronchoscopy. Conclusion Local anesthesia with 1% concentration of lidocaine shares the similarly anaesthetic effect with 2% concentration of lidocaine on bronchoscopy.
