摘 要:目的肾上腺素对血流动力学的影响主要与肾上腺素剂量、输注速度及输注途径有关。本研究观察体外循环(cardiopulmonary bypass,CPB)前静脉注射及CPB中储血器中加入小剂量肾上腺素对大鼠血流动力学的影响。方法成年雄性SD大鼠36只建立CPB模型,根据给药方式不同随机均分为3组(n=12):ES组CPB前静脉注射肾上腺素0.5μg/kg,CPB过程中储血器中加入等容等渗盐水0.4 ml;SE组CPB前静脉注射等容等渗盐水0.4 ml,CPB过程中储血器中加入肾上腺素0.5μg/kg;SS组CPB前,CPB过程中均给予等渗盐水0.4 ml。记录CPB前及CPB过程中,肾上腺素或等渗盐水注射时0 s(基础值)和注射后5、10、15、30 s及1、2、3、4、5 min时的平均动脉压(mean arterial pressure,MAP)和心率(heart rate,HR)。结果 CPB前,与基础值相比,SS组和SE组各时间点MAP未见明显改变;ES组MAP 5 s时上升,15 s和30 s时下降(P<0.05),1 min时恢复至基础值水平。CPB中,与基础值相比,SS组ES组各时间点MAP未见明显改变,SE组MAP 15 s至1 min时MAP上升(P<0.05)。结论 CPB前大鼠静脉注射0.5μg/kg肾上腺素可引起MAP出现先升后降的双相变化,CPB过程中储血器中加入0.5μg/kg肾上腺素可引起大鼠MAP出现明显的上升。Objective Hemodynamic effect of epinephrine is associated with its dose, infusion rate and route of administra- tion. The present study was to observe the hemodynamic effect of low-dose epinephrine intravenously injected before cardiopulmonary bypass (CPB) and added in blood reservoir during CPB. Methods We established a model of CPB in 36 SD male rats and random- ized them into 3 groups of equal number : an ES group, intravenously injected with epinephrine 0.5 μg/kg before CPB and saline 0.4 mL added in blood reservoir during CPB ; an SE group, injected with saline 0.4 mL before CPB and epinephrine 0.5 μg/kg added in blood reservoir during CPB; and an SS group: injected with saline 0.4 mL before CPB and added in blood reservoir during CPB. We recorded the mean arterial pressure (MAP) and heart rate (HR) at 0 s ( baseline), 5 s, 10 s, 15 s, 30 s, 1 min, 2 min, 3 min, 4 rain and 5 min after the injection of epinephrine or saline before and during CPB. Results Before CPB, MAP showed no significant chan- ges in the SS and SE groups but increased at 5 s (P 〈0.05), decreased at 15 s and 30 s (P 〈0.05), and returned to the baseline level at 1 min in the ES group as compared with the baseline. During CPB, MAP exhibited no signifi cant changes in the SS and ESgroups but increased from 15 s to 1 min in the SE group in comparison with the baseline (P 〈 0.05). Conclusion Intravenous injection of epinephrine 0. 5 μg/kg elicited biphasic changes in MAP, first increased and then decrease before CPB. Epinephrine O. 5 μ/kg added in blood reservoir induced an obvious increase in MAP during CPB.
