摘  要:目的应用血管回声跟踪(echo-tracking,ET)技术评价冠心病患者颈动脉早期硬化,以预测冠状动脉早期病变。方法应用ET技术测量比较40例冠心病患者及31例冠心病高危组与48例正常对照组的颈总动脉的弹性指标,包括压力硬度指数(β)、压力应变弹性系数(Ep)、顺应性(AC)、增大指数(AI)和脉搏波传导速度(PWVβ)。并对冠心病组进行冠状动脉造影,定量检测其颈总动脉弹性指标与冠状动脉狭窄程度比较。结果冠心病组及冠心病高危组的颈总动脉Ep、β、AI、PWVβ测值与正常组相比显著增高(P<0.05),AC低于对照组,但差异无统计学意义(P>0.05)。冠状动脉狭窄程度越重颈总动脉Ep、β、AI、PWVβ就越高,顺应性AC就越低,差异有统计学意义。双支及多支冠状动脉病变组颈总动脉Ep、β、AI、PWVβ均高于单支组及无病变组,而AC低于无病变组,差异有统计学意义(P<0.05)。结论 ET成像技术为冠心病患者冠状动脉早期硬化趋势提供了一种快速、无创、定量的评价方法。Objective To predict coronary artery early lesions by evaluating the stiffness of carotid artery by echo tracking. Methods Forty patients with CHD(Coronary Heart Diseease)and thirty-one patients with CHD risk group and Forty-eight control subjects were recruited in this study. They also under went ET technique for homodynamic quantification to evaluate the blood vessel elasticmodulus, such as, pressurest rain elasticmodulus (Ep) stiffness parameter (β), arterial compliance(AC), arterial increase the index (AI) and pulse wave velocity (PWVβ). Coronary arteriography was tested For CHD group, then compare carotid artery elasticity index with coronary artery stenosis. Results Carotid artery Ep, 13, AI and PWV13 measured values of coronary heart disease group and coronary heart dis ease risk group were significantly higher than the normal group (P〈0.05) ,their AC was lower than the norma group, but the difference was not statistically significant(P〈0.05). Coronary artery stenosis is the more severe the carotid artery elasticity index Ep,β, AI, PWVβ are higher and AC is lower, The elasticity (Ep, β, AI, PWVβ)of double and multivessel disease carotid artery are higher than the single group and no lesion group, their AC is lower than no lesion group, the difference statistically significant (P〈0.05). Conclusions ET for coronary heart disease in patients with coronary artery early hardening trend provides a fast, noninvasive, quantitative evaluation methods.
