摘  要:目的观察子宫切口妊娠应用高强度聚焦超声(HIFU)热辐照治疗后的临床效果,为临床治疗子宫切口妊娠提供一种安全、简便、有效的方法。方法 10例经临床确诊并自愿接受HIFU治疗的子宫切口妊娠(CSP)患者用HIFU对妊娠囊进行热辐照2~3次,1次/d,同时彩超观察孕囊大小、周边血流、阻力指数及心管搏动等变化,治疗前、治疗后每周测血清β-HCG值直到正常。结果 10例CSP患者治疗过程中生命体征平稳,治疗后7 d病灶周边血流明显减少,阻力指数增加,病灶内可见液化区,妊娠囊均有不同程度的缩小,血β-HCG值以50%~70%/周的速度逐渐下降。其中7例治疗前可见明显心管搏动者治疗后心管搏动即刻消失,4~5周后恢复正常月经周期,并伴坏死的妊娠囊组织脱落排出,3例5周后恢复正常月经周期。结论用HIFU热辐照治疗CSP是一种比较安全、有效、简便的治疗方法。Objective To observe the clincical efficacy of highintensity focused ultrasound(HIFU)) for the treat ment of caesarean scar pregnancy(CSP)patients. Methods Ten cases of clinically diagnosed CSP were performed by HIFU 2 or 3 times a day, the volume of gestatinal sac, blood flow surrounding geatatonal sac, resistance index(RI) and beat of primary cardiac tube before and after HIFU were compared by Color Doppler Flow Imaging(CDFI). Seum lev- els of Beta Human Chorionic Gonadotrophin(Beta -HCG) was measured weekly until it restored to normal levels. Resuits All of the ten patients were hemodynamically stable during HIFU treatment, the non-echo structure in sac appeared and the volume of gestatinal sac decreased, blood flow surrounding geatatonal sac weaken or disappeared, resist ance index(RI) increased and beat of primary cardiac tube disappeared soon after HIFU in seven of ten patients. Seum levels of Beta Human Chorionic Gonadotrophin(Beta -HCG) restored by 50 to 70 percentage per week. The menstrual cycle returned to normal in four to five weeks with passage of necrotic tissue. Conclusions HIFU is safe, effective and simple method for the treatment of caesarean scar pregnancy.
