鼻内镜双径路手术治疗真菌性上颌窦炎

摘　　要：目的探讨鼻内镜双径路手术治疗非侵袭性真菌性上颌窦炎的效果。方法对43例非侵袭性真菌性上颌窦炎采用鼻内镜下中鼻道联合下鼻道双径路手术治疗,术后处理同普通慢性鼻窦炎,未全身或局部使用抗真菌药物。结果术后6个月,43例患者临床症状消失,鼻腔、中鼻道无分泌物,窦口引流通畅,鼻内镜检查窦腔内黏膜正常,未见异常分泌物及真菌团块存留,未发生任何并发症。随访1～5年未见复发。结论鼻内镜下中鼻道联合下鼻道双径路手术是治疗非侵袭性真菌性上颌窦炎的有效方法,术后定期随访及窦腔冲洗可防止复发,无须使用抗真菌药物。Objective Objective To evaluate the effect of endoscopic nasal surgery with dual approaches on noninvasive fungal maxillary sinusitis.Methods Forty-three patients with noninvasive fungal maxillary sinusitis were treated with endoscopic maxillary sinus surgery combined with windowing of inferior nasal meatus.The postoperative treatment was the same as the treatment of common chronic rhinosinusitis.No systemic or topical treatments with antifungal drugs were performed after operation.Result The clinical symptoms disappeared in all patients 6 months after operation.NO secretions were found in middle nasal meatus and nasal cavity.The sinus mucous membrane was normal and ostial drainage was adequate.In addition,there was no abnormal discharge or fungal mass in sinus cavity.Neither complication nor recurrence occurred during the 1-5 year period of follow-up.Conclusion Endoscopic maxillary sinus surgery combined with windowing of inferior nasal meatus is an effective treatment for noninvasive fungal maxillary sinusitis.The regular irrigation and follow-up after operation are important for avoiding recurrence.Antifungal drugs are not necessary for noninvasive fungal maxillary sinusitis.
