摘 要:目的研究免疫营养素强化的早期肠内营养对脓毒症大鼠肠道免疫功能的影响。方法雄性SD大鼠20只,在盲肠结扎并穿刺后按随机数字表法分为普通肠内营养组(EN组,n=10)和肠内免疫营养组(EIN组,n=10)。EN组灌喂普通肠内营养液(EN),EIN组灌喂等氮等热量的谷氨酰胺和精氨酸的强化的肠内营养液(EIN)。2组营养剂糖、脂肪、蛋白质供热比为82%∶3%∶15%,卡氮比为150∶1。每天每只大鼠营养摄入按732.2kJ.kg-1.d-1标准计算,2组营养摄入为等氮、等热量、等体积。连续灌喂7d后处死全部大鼠,取全段小肠,计数总的肠集合淋巴结(Peyer’s patches,PP)细胞数,流式细胞仪检测PP结的T和B细胞的变化,双抗体夹心酶联免疫吸附实验(ELISA)检测肠IgA的变化。结果与普通肠内营养组相比,EIN组总的PP细胞数、T细胞和B细胞数、肠IgA水平均高于EN组,差异均有统计学意义(均P<0.05)。结论免疫营养素强化的早期肠内营养可以改善脓毒症大鼠肠道免疫功能。Objective To investigate the effects of early enteral immunonutrition on intestine immune function in septic rats.Methods After sepsis was induced by cecal ligation and puncture(CLP),20 male SD rats were randomly assigned into two groups,with 10 rats in each group.EN group was given conventional enteral nutrition with a liquid formula.EIN group was given glutamine and arginine-enriched enteral nutrition.The relative proportions of energy in both groups were 82% carbohydrate,3% fat and 15% protein with a ratio of calorie to nitrogen of 150∶1.The energy requirement of each rat was calculated according to 732.2 kJ·kg-1·d-1.Rats received isocaloric,isonitrogenous,and isovolumic enteral nutrition in both groups.All rats were killed after continuous feeding for 7 days.Entire intestine was collected and total cell count in Peyer's patches was determined.The numbers of T and B cells in Peyer's patches were measured by flow cytometry.The intestinal immunoglobulin A(IgA) levels were detected by ELISA.Result Total cell yield,numbers of T and B cells in Peyer's patches,and intestinal IgA levels in EIN group were significantly higher than those in EN group(P<0.05).Conclusion Early enteral immunonutrition can improve intestine immune function in septic rats.
