摘 要:目的探析孕妇血小板抗体与新生儿血小板减少之间的相关性,为临床预防婴儿血小板减少性相关疾病提供指标。方法选取在我院妇产科待产且经检查患血小板减少的孕妇76例作为观察组,血小板正常的孕妇78例作为对照组,采用流式细胞仪技术检测观察组孕妇血小板表面抗体,用酶联免疫吸附法(ELISA)检测观察组孕妇血小板膜糖蛋白特异性自身抗体,并对两组新生儿进行血小板计数。结果观察组孕妇所生婴儿血小板减少发生率为38.16%,远高于血小板正常的健康孕妇所生婴儿血小板减少发生率(6.41%),P<0.05,差异有统计学意义。观察组孕妇血小板表面抗体阳性检出率(53.95%)明显比血小板膜糖蛋白特异性自身抗体阳性检出率(27.63%)高,而血小板表面抗体检测敏感度为41.46%,远低于血小板膜糖蛋白特异性自身抗体检测敏感度(95.24%),经χ2检验,P<0.05,差异有统计学意义。结论新生儿血小板减少与孕妇患血小板减少性相关疾病有关,孕妇血小板抗体检测有望成为预防新生儿血小板减少的检测指标。Objective To explore and analyze the correlation between platelet antibody and neonatal throm-bocytopenia in pregnant women, and to provide guidance for the clinical prevention of neonatal thrombocytopenia re-lated diseases. Methods Seventy-six pregnant women with thrombocytopenia waiting for delivery at Department of Gynaecology and Obstetrics in our hospital were enrolled as the study group. Seventy-eight pregnant women with nor-mal thrombocyte were selected as the control group. Flow cytometry was adopted to detect and observe the platelet surface antibody, and enzyme-linked immunosorbent assay (ELISA) was used to test and observe the platelet mem- brane glycoproteins specific autoantibody. The newborns' platelets were counted. Results Newborns born by the women in the study group had significantly higher incidence of thrombocytopenia (38.16%) than those born by the women in the control group (6.41%), P〈0.05. The positive detection rate of platelet surface antibody in the study group was 53.95%, significantly higher than the positive detection rate of platelet membrane glycoproteins specific au- toantibody (27.63%). The detection sensitivity of platelet surface antibody in the study group was 41.46%, significant-ly lower than the detection sensitivity of platelet membrane glycoproteins specific autoantibody (95.24%), P〈0.05. Conclusion Neonatal thrombocytopenia showed a significant correlation with the thrombocytopenia related diseases in pregnant women. Detection ofplatelet antibody for pregnant women is expected to become the testing index for pre-vention of neonatal thrombocytopenia.
