摘  要:目的应用超声心动图评价室间隔缺损(VSD)患者封堵术前与术后的左室形态及收缩、舒张功能的变化。方法回顾性分析遵义医学院附属医院确诊为VSD患者33例。所有患者于封堵术前1 d及术后3 d、1月、3月、5月分别测量VSD患者左房前后径(LAD)、左室舒张末期前后径(LVEDD)、左室舒张末期长径(LVEDLD)、左室舒张末期中间段最大横径(LVEDMD),二尖瓣口舒张期血流E峰、A峰速度,左室游离壁侧二尖瓣环收缩期运动速度(s)、舒张早期运动速度(e)、舒张晚期运动速度(a)及左心室射血分数(LVEF)。结果①左室术前术后形态变化:术后3 d、1月、3月、5月LVEDD、LVEDLD、LVEDMD与术前比较逐渐回缩,差异有统计学意义(P<0.05);术后1月、3月、5月LAD测量值与术前及术后3d比较均逐渐回缩,差异有统计学意义(P<0.05)。②左室术前术后功能变化:术后3 d、1月、3月、5月E峰速度、A峰速度,s、e、a测值均较术前明显降低(P<0.05);术后3d、1月、3月、5月LVEF术后虽有所下降,但差异无统计学意义(P>0.05)。结论 VSD封堵术不仅能纠正心脏的解剖畸形,同时能纠正其血流动力学改变,使得左室形态及功能得到逆转并逐渐恢复至正常水平。Objective To assess the changes left ventricles before and after the transcatheter in the form, volume, contraction and diastolic function of closure of Ventricular Septal Defect (VSD) through ech- ocardiography. Methods Thirty - three cases diagnosed with VSD in the Affiliated Hospital of Zunyi Medical College were analyzed retrospectively. The left atrial diameter (LAD) ,left ventricular end - di- astolic dimension (LVEDD),left ventricular end -diastolic long diameter (LVEDLD), left ventricular end - diastolic middle period of maximum diameter ( LVEDMD), E peak and A peak of mitral diastolic blood flow, left ventricular systolic movement speed (s) and early diastolic movement speed (e) and di- astolic late movement speed (a) and left ventricular ejection fraction (LVEF) were measured 1 day be- fore the transcatheter closure and 3 days, 1,3 and 5 months after the transcatheter closure, respectively. Results ①Compared with the pre - operation, the LVEDD, LVEDLD, LVEDMD significantly gradually lessened 3 days, 1 month,3 months and 5 months after the transcatheter closure. Compared with the pre - operation and 3 days after operation, the LAD significantly gradually lessended 1,3 and 5 months after the transcatheter closure. ② Compared with the pre - operation, the E peak rate, A peak rate, s, e and a significantly decreased 3 days, 1,3 and 5 months after the operation. However, the LVEF declined 3 days, 1,3 and 5 months after operation but no significant difference among these groups was detected.Conclusion The transcatheter closure of VSD could not only correct the heart anatomic deformity and the hemodynamic changes, but also make the shape and function of left ventricular reverse and return to the normal level.
