摘 要:目的探讨人脐带间充质干细胞(human umbilical cord-derived mesenchymal stem cells,hUC-MSC)移植在防治兔肝内缺血性胆道病变中的作用。方法将18只新西兰大白兔随机分成3组(每组6只):(1)A组(假手术组),仅切开腹腔,不予任何特殊处理,术后分层关腹;(2)B组(实验模型组),建立肝内缺血性胆道病变模型;(3)C组(hUC-MSC移植组),术前12h经耳缘静脉注射hUC-MSC(2×106/kg)5ml,然后按B组实验操作建立肝内缺血性胆道病变模型。术后定时监测生化指标变化情况。术后4周开腹行胆道造影。处死动物后,取第一肝门处汇管区肝组织做病理学检查。结果与A组比较,B组和C组术后多个时间点各项生化指标均有不同程度升高(均为P<0.05),但C组的生化指标升高程度明显低于B组(均为P<0.05),下降趋势明显。术后4周胆道造影、病理学检查结果示,B组的肝内胆管上皮细胞破坏严重,C组的胆管上皮细胞破坏较B组明显减轻,管壁结构完整。结论术前hUC-MSC移植能够有效减轻缺血性胆道病变的程度,预防缺血性胆道病变的发生。Objective To investigate effect of human umbilical cord-derived mesenchymal stem cells (hUC-MSC) transplantation on preventing ischemic-type biliary lesions in rabbit liver. Methods Eigh- teen New Zealand white rabbits were randomly divided into three groups ( n = 6 in each). ( 1 ) Group A (sham-operation group): abdominal cavity was only cut open without any special treatment and sewed up layeredly after operation. (2) Group B (experimental model group) : models of ischemic-type biliary lesions in liver were established. (3) Group C (hUC-MSC transplantation group) : hUC-MSC (2 x 106/kg, 5 ml) was injected by ear margin veins at 12 h before operation and models of ischemic-type biliary lesions in liver were established according to the experimental operation in Group B. Changes of biochemical markers were monitored regularly after operation. Cholangiography was performed by cutting open the abdominal cavity at 4 weeks after operation. After executing animal, pathological examination was performed in the liver tissue of portal area at the first porta hepatic. Results Compared with Group A, the biochemical markers of Group B and C increased at different degree at several time points after operation ( all in P 〈 0. 05 ). But the elevated degree of biochemi- cal markers in Group C was lower than that in Group B ( all in P 〈 0. 05). The results of cholangiography and pathological examination showed that epithelial cells of intrahepatic biliary were destroyed in Group B. And epi- thelial cells of intrahepatic biliary lesion in Group C was more alleviative than that of Group B with intact bile duct wall. Conclusions hUC-MSC transplantation before operation can lessen ischemic-type biliary lesions and prevent ischemic-type intrahepatic biliary lesion.
