愈创方运用蒸发罨包疗法促进混合痔创面愈合的临床疗效观察

摘　　要：目的:研究愈创方运用蒸发罨包疗法促进混合痔创面愈合及改善痔术后并发症的临床疗效。方法:将符合纳入标准的116例混合痔术后患者随机分为两组,愈创方组(Yuchuang Recipe Group,YRG)59例,红外线组(Infrared Ray Group,IRG)57例。再根据愈创方按当归10g、赤芍10g、徐长卿10g、芒硝10g的水煎剂进行罨包的制作,同时准备好红外线治疗仪及生肌散。术后两组行相同的基础治疗,再每天两次使用生肌散换药后,蒸发罨包组予45℃半干罨包一块外敷肛缘,红外线组予红外线治疗仪照射肛缘创面10分钟。观察并记录两组患者术后连续7天的创面疼痛、肛缘水肿、创面渗液的积分及创面愈合天数等几项指标。结果:愈创方组在减轻创面疼痛方面,术后1d至3d,疗效相当于红外线组(P>0.05),术后4d至7d,疗效优于红外线组(P<0.05);愈创方组在消除肛缘水肿方面,术后1d至7d,疗效等于红外线组(P>0.05);在减少创面渗液方面,术后1d至3d,两组疗效相当(P>0.05),术后4d至7d,愈创方组疗效优于红外线组(P<0.05);愈创方组在促进创面愈合方面,疗效优于红外线组(P<0.05)。结论:愈创方在缓解混合痔术后并发症及促进创面愈合的疗效方面具有明显的优势。Objective： To investigate the clinical effect of Yuchuang Recipe with evaporated chliasma sack in improving postoperative complications of mixed hemorrhoids and promoting postoperative wound healing.Methods： One hundred and sixteen postoperative mixed hemorrhoids patients consistent with the test criteria were randomly and equally divided into two groups： Yuchuang Recipe Group and Infrared Ray Group.And according to the Yuchuang Recipe angelica 10g,Radix 10g,Xu Changqing 10g,mirabilite 10g of water decoction to foment the production of the package.While infrared therapy equipment and shengji san was got ready for trial.First,the same steps were given for two groups.Then each group will be changed dressings twice every day after operation.The Yuchuang Recipe Group will be spreaded a piece of damp-dry 45℃ evaporated chliasma sack on the edge of anus;The Infrared Ray Group will be irradiated by the infrared therapy equipment ten minutes.The integral of wound pain,edema,oozing fluid will be obversed and recorded in the continuous seven days and wound healing dayswill be recorded after operation.Results： In the aspect of releasing wound pain,the Yuchuang Recipe Group equal to Infrared Ray Group from the first day to the third day after operation（P0.05）;The Yuchuang Recipe Group was better than the Infrared Ray Group from the forth day to the seventh day（P0.05）.In the aspect of eliminating edema of the edge of anus,the Yuchuang Recipe Group was equal to the Infrared Ray Group from the first day to the seventh day（P0.05）.In the aspect of diminishing oozing fluid,the two groups have the same therapeutic equivalence from the first day to the third day;The Yuchuang Recipe Group was better than the Infrared Ray Group from the forth day to the seventh day（P0.05）.In the aspect of promoting wound healing,the Yuchuang Recipe Group was better than the Infrared Ray Group（.P0.05）.Conclusion： There are obvious advantages of Yuchuang Recipe in improving postoperative complications of mixed hemorrhoids
