血栓抽吸导管联合盐酸替罗非班在急诊冠状动脉介入治疗中的应用

摘　　要：目的:观察急性心肌梗死患者行急诊冠状动脉介入治疗(PCI)过程中联合应用血栓抽吸导管和盐酸替罗非班的有效性与安全性。方法:选择2009年1月—2011年10月符合急诊PCI治疗的急性ST段抬高心肌梗死患者共128例,以随机抽样法分为观察组与对照组各64例。观察组给予血栓抽吸与盐酸替罗非班,对照组给予盐酸替罗非班完成PCI术。观察2组患者术前、术后心肌梗死溶栓试验(TIMI)血流情况、住院期间左心室射血分数(LVEF)、1个月内主要不良心脏事件(MACE)发生率以及出血并发症发生率。结果:观察组住院期间LVEF、术后即刻TIMI血流情况高于对照组,1个月内MACE发生率和出血并发症发生率无显著差异。结论:在急性心肌梗死患者行急诊冠状动脉介入治疗过程中联合应用血栓抽吸导管和盐酸替罗非班安全可行,可降低无复流发生率,无致命性出血并发症,是安全、有效的治疗方法。OBJECTIVE：To evaluate the efficacy and safety of thrombus aspiration plus tirofiban applied during emergency percutaneous coronary intervention（PCI） in patients with acute myocardial infarction.METHODS：Ninety emergency PCI candidates with acute myocardial infarction and ST segment elevation from January 2009 to October 2011 were randomly assigned to receive either trial group（thrombus aspiration plus tirofiban） or control group（tirofiban） of 64 each.The TIMI（thrombolysis in myocardial infarction） blood flow before and after operation,the left ventricular ejection fraction（LVEF） during hospitalization and incidence of MACE（major acute coronary event） and incidence of bleeding complication etc within one month after operation were recorded for the two groups.RESULTS：The LVEF during hospitalization and the TIMI flow instantly after operation in the trial group were significantly higher than in the control group;however,the incidences in MACE and bleeding complication were not significantly different between the two groups.CONCLUSION：Thrombus aspiration plus tirofiban applied during emergency PCI was proved to be safe and effective for acute cardiac infarction in that it can reduce the incidence of no-reflow yet without presence of fatal complication of bleeding.
