摘　　要：目的:探讨临床应用注射剂的各种风险、临床药师可以进行干预的节点及方法,确保注射剂的安全应用。方法:通过检索国内外文献,结合临床药师自身实践,分析、总结注射剂临床应用过程中的各种风险因素,对医、护人员进行教育与指导、考核、直接进行药学干预等措施,保证用药安全。结果:临床药师参与注射剂安全应用风险管理,能切实避免或减少由于注射剂导致的不良反应,将注射剂给药的失误和相关不良反应降至最低,提高了用药安全性。结论:对注射剂应用过程中的各种风险因素应进行有效管理,临床药师不仅有能力、而且有责任参与其风险管理,这既是融入治疗团队的有效切入点,也是进一步促进临床合理用药的有效出发点。OBJECTIVE：To analyze the risks in clinical use of injections and the entry point and approaches for clinical pharmacists to perform intervention on drug use so as to ensure the safe use of injections.METHODS：The pertinent literature both at home and abroad was reviewed.The risk factors in clinical use of injections noted by clinical pharmacists in clinical medication were analyzed and summarized,and education and guidance and evaluation on medical staff as well as direct pharmaceutical intervention were performed by clinical pharmacists to ensure medication safety.RESULTS：Clinical pharmacists＇ involvement in risk management on safe use of injections prevented or reduced the incidence of adverse drug reactions induced by injections,minimized the incidences of the faults in the administration of injections and the related adverse drug reactions and improved medication safety.CONCLUSION：It is necessary to perform effective management on risk factors in clinical use of injections.Clinical pharmacists have the capability and responsibility to engage in the risk management,which is an effective entry point for clinical pharmacists to be one of the members of the treatment team as well as an effective starting point for promoting clinical safe and rational drug use.
