摘  要:目的探讨后路开窗减压、伤椎和相邻节段椎弓根钉固定治疗胸腰椎爆裂性骨折的效果。方法选择35例胸腰椎爆裂性骨折患者,通过开窗减压或半椎板减压,在伤椎和正常椎体植入椎弓根钉固定,术后定期记录椎体高度、神经功能和后突角变化情况。结果 35例胸腰椎爆裂性骨折患者术后随访1~5年,伤椎骨愈合时间为2~4个月,术后6个月骨折完全愈合、神经功能明显改善、术后椎体高度未见明显丢失、Cobb’s角较术前明显改善,Frankel分级有好转。结论后路开窗减压、伤椎短节段固定治疗胸腰椎爆裂性骨折安全、有效。Objective To explore the effect of the treatment of thoracolumbar burst fractures through the posterior approach fenestration or semi-laminectomy decompression and fixation by embedding pedicle screws between the injured and normal vertebrae. Methods 35 cases of thoracolumbar burst fractures patients were selected. After fenestration or semilamineetomy decompression, the adjacent injured-vertebrae and normal vertebras were fixed by embedding pedicle screws. In order to evaluate the safety and effectiveness of this method, the injured-vertebral heights, neurological functions and the after process angles were recorded regularly after the surgery. Results 35 cases of the patients were followed for 1 to 5 years, 2 to 4 months after the surgery, the fracture recovered markedly, neurologic function improved markedly and no obvious vertebral body height loss after the restoration during a review of X-ray, Cobb's horns angle improved obviously compared with anterior-operation, Frankel classification improved also. Conclusions Through the posterior fenestration or semiaminectomy decompression, the treatment of tboracolumbar burst fractures through fixation by embedding pedicle screws into the injured-vertebrae and normal vertebrae is safe and effective.
