透刺法治疗顽固性周围性面瘫眼睑闭合不全临床观察

摘　　要：目的观察透刺法治疗顽固性周围性面瘫眼睑闭合不全的疗效。方法 55例顽固性周围性面瘫眼睑闭合不全的患者均采取透刺法。治疗1个疗程及2个疗程后观察面瘫整体疗效;治疗前、治疗1个疗程及2个疗程后,测量用力闭眼后上下眼睑的距离及采用手法肌力检查方法评定肌力。结果透刺法治疗顽固性周围性面瘫眼睑闭合不全,总有效率为74.5%;治疗1个疗程及治疗2个疗程上下眼睑距离与治疗前比较,经配对t检验,均P<0.05,提示针刺不同疗程均能减少上下眼睑距离;治疗2个疗程上下眼睑距离与治疗1个疗程比较,经配对t检验,均P<0.05,提示治疗2个疗程后上下眼睑距离减少更明显。治疗1个疗程及治疗2个疗程眼睑肌力与治疗前比较,经配对t检验,均P<0.05,提示针刺不同疗程均能提高眼睑肌力。结论透刺法对顽固性周围性面瘫眼睑闭合不全有明显改善作用。Objective To investigate the therapeutic effect of point-to-point acupuncture on lagophthalmos in refractory peripheral facial paralysis. Method Fifty-five lagophthalmos patients with refractory peripheral facial paralysis were all treated with point-to-point acupuncture. The overall therapeutic effect on facial paralysis was evaluated after one and two courses of treatment. The distance between upper and lower eyelids was determined after forced eye closure and the myodynamia was measured by manual muscle testing before treatment and after one and two courses of treatment. Result A paired t-test showed a significant difference in the distance between upper and lower eyelids between before treatment and after 10 or 20 days of treatment for lagophthalmos in refractory peripheral facial paralysis （both P〈0.05）, suggesting that different courses of acupuncture treatment could decrease the distance between upper and lower eyelids. A paired t-test showed a significant difference in the distance between upper and lower eyelids between 10 and 20 days of treatment （P〈 0.05）, suggesting that the distance decreased more after 20 days of treatment. A paired t-test also showed a significant difference in the strength of the eyelid muscles between before treatment and after 10 or 20 days of treatment （both P〈 0.05）, suggesting that different courses of acupuncture treatment could increase the strength of the eyelid muscles. Conclusion Point-to-point acupuncture has a marked improving effect on lagophthalmos in refractory peripheral facial paralysis.
