普罗布考对早期糖尿病肾病患者血清超敏C反应蛋白和炎性因子表达的影响

摘　　要：目的观察普罗布考对早期糖尿病肾病患者血清超敏C反应蛋白(hs-CRP)和炎性细胞因子白细胞介素1(IL-1)、白细胞介素6(IL-6)水平的影响。方法将74例患者随机分为常规治疗组(A组)和普罗布考组(B组)。B组在常规治疗基础上加用普罗布考500 mg/次,每日2次,疗程8周。比较两组患者治疗前后血糖、血脂、24 h尿微量白蛋白排泄率(UAER)、hs-CRP及血清IL-1、IL-6水平。结果治疗前两组血清总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL-C)、低密度脂蛋白(LDL-C)、空腹血糖(FBG)、血尿素氮(BUN)、血肌酐(SCr)、UAER,hs-CRP,IL-1和IL-6水平比较,差异无统计学意义(P>0.05)。治疗第8周后,A组与治疗前比较,TC,TG,LDL-C,HDL-C,UAER,hs-CRP和IL-1水平无显著变化(P>0.05);B组与治疗前比较,TC,TG,LDL-C,UAER,hs-CRP,IL-1和IL-6水平显著下降(P<0.05或P<0.01)。与A组比较,B组治疗8周后TC,TG,LDL-C,UAER,hs-CRP,IL-1和IL-6水平显著下降(P<0.05或P<0.01)。结论普罗布考在发挥降脂作用的同时,可显著降低早期糖尿病肾病患者血清hs-CRP及炎症介质IL-1,IL-6的水平,延缓肾损害。Objective To observe the effect of probucol on the serum levels of hs-CRP and inflammatory cytokines IL-1 and IL-6 in the patients with early-stage diabetic nephropathy（DN）.M ethods 74 patients with early-stage DN were randomly divided into two groups,the routine treatment group and the probucol group.The probucol group was given probucol 500 mg,twice daily for 8 weeks,on the basis of the conventional treatment.The serum levels of glucose,lipid,UAER,hs-CRP,IL-1 and IL-6 before and after treatment were measured and compared between the two groups.Results There were no significant differences in the serum levels of TC,TG,HDL-C,LDL-C,FBG,BUN,SCr,UAER,hs-CRP,IL-1 and IL-6 between two groups before treatment（P 0.05）.After 8-week treatment,the serum levels of TC,TG,LDL-C,HDL-C,UAER,hs-CRP and IL-1 showed no obvious changes compared with those before treatment in the routine treatment group（P 0.05）,while the serum levels of TC,TG,LDL-C,UAER,hs-CRP,IL-1 and IL-6 in the probucol group were decreased significantly（P 0.05 or P 0.01）.Compared with the routine treatment group,the serum levels of TC,TG,LDL-C,UAER,hs-CRP,IL-1 and IL-6 after 8-week treatment in the probucol group were decreased significantly（P 0.05 or P 0.01）.Conclusion Besides playing lipid-lowering effect,probucol can significantly decrease the serum levels of hs-CRP and inflammatory mediators IL-1 and IL-6 in early-stage DN and delay the renal damage of DN.
