摘 要:【目的】探讨腰围身高比值(WHtR)作为肥胖筛查指标在小学体检中应用的可行性。【方法】采用分层整群随机抽样法,选取秦皇岛市7~12岁学生1507人(男763人,女744人)作为研究对象,测量身高、体重和腰围(wC),计算体质指数(BMI)和WHtR。【结果]WHtR与年龄无相关(男生r=-0.008,女生r=0.005,P〉0.05)。男生WHtR与BMI(r=0.879)和WC(r=0.892)呈正相关(P〈0.01)。女生WHtR与BMI(r=0.779)和wC(r=0.837)呈正相关(P〈0.01)。WHtR识别儿童超重肥胖的ROC曲线下面积均在0.9以上(P〈0.01),准确性高。【结论】WHtR作为相对简单的测量方法,可有效识别儿童超重肥胖,便于校医操作,应列为学生体检的常规测量项目。[Objective] To explore the feasibility of waist to height ratio(WHtR) as the screening index of obe- sity in health examination. [Methods] The stratified cluster random sampling method was used to choose I507 students aged 7-12 years old(763 boys and 744 girls) in Qinhuangdao city as the study subjects. Height, weight and waist circumference(WC) were measured. Body mass index(BMI) and WHtR were calculated. [Results] There was no correlation between WHtR and age(boys r=-0. 008 and girls r=0. 005, P 〉0.05). WHtR showed a strong positive correlation with BMI(boys r=0. 879 and girls r=0. 779) and WC(boys r=0. 892 and girls r= 0. 837)( P〈0.01). The accuracy levels of WHtR for identifying overweight and obesity(as assessed by area un- der the curve) were both over 0.9( P〈0.01). [Conclusion] WHtR is a relatively easy measurement method, and can effectively identify overweight and obesity in children, and is convenient to be used by school doctors. There- fore, WHtR should be used as the routine measurement item of physical examination for schoolchildren.
