18F—FDGPET／CT显像在结直肠癌术后复发和转移诊断中的应用

摘　　要：【目的】探讨18F-脱氧葡萄糖-正电子发射型计算机断层显像与CT同机融合显像（18F-FDGPET／CT）在早期诊断结直肠癌术后复发、转移中的{晦床应用价值。【方法】选取2005年5月至2010年12月在四川省人民医院行结直肠癌手术后的患者共21例，其中可疑复发、转移10例（疑发组）和明确复发、转移11例（复发组）。术后均行全身18F_FIXgPET／CT检查。将其诊断结果与最终诊断结果进行对照，计算18F_FIXgPET／CT诊断的灵敏度、特异性、准确性、阳性预测值及阴性预测值，最后分析18F-FDGPET／CT检查前、后患者治疗方案的更改情况。【结果】18F-FDGPET／CT正确诊断19例，其中疑发纽8例，复发纽11例；1例假阳性，l例假阴性。18F-FIX／PET／CT对术后复发、转移诊断的灵敏度、特异性、准确性、阳性预测值及阴性预测值分别为92．3％、90．0％、91．3％、92．3％及90．0％。5例再分期及治疗方案发生更改。【结论】”F-FDGPET／CT检查在结直肠癌患者术后复发、转移的诊断中具有重要价值。[Objective] To explore the clinical value of 18 F-deoxyglusoce-positron emission tomography/computed torno graphy（18 F FDGPET/CT） in the early diagnosis of recurrence and metastasis of coloreetal cancer after operation. [Meth- ods] Twenty one patients with eolorectal cancer after operation in Sichuan provincial people＇s hospital from May 2005 to Dec. 2010 were chosen and divided into suspected recurrence and metastasis group（n= 10, suspected group） and identified recurrence and metastasis group（n= 11, recurrence group）. ~F-FDGPET/CT was performed for all patients after opera- tion. The diagnosis result was compared with the final diagnosis result. The sensitivity, specificity, accuracy, positive pre- dictive value（PPV） and negative predictive value（NPV） of 18F-FDGPET/CT were calculated. At last, the change of treat- ment protocols of patients before and after PET/CT examination was analyzed. [Results]Nineteen patients were correctly di- agnosed by 18 F-FI）GPET/CT, in which 8 cases were in suspected group and 11 cases in recurrence group. One patient was false positive and one patient was false negative. The sensitivity, specificity, accuracy, PPV and NPV of 18 F-FIXiPET/CT for the diagnosis of the recurrence and metastasis were 92.3%, 90.0%, 91.3%, 92.3% and 90.0%, respectively. Five patients were staged again, and their treatment protocols were changed. [Conclusion] 18F FDGPET/CT has an important value in the diagnosis of recurrence and metastasis of colorectal cancer after operation.
