摘 要:目的探讨早期康复训练对早产儿发育的影响。方法选取70例早产儿,男孩37例,女孩33例,随机分成实验组和对照组各35例,两组患儿性别、分娩方式、围产期情况、父母年龄、文化程度、职业经济状况以及居住环境相似,实验组胎龄(34.70±2.26)周,出生体重(2.38±0.60)kg,Apgar评分(4.60±0.80),对照组胎龄(35.65±1.96)周,出生体重(2.68±0.82)kg,Apgar评分(4.86±0.46),两组差异无统计学意义;实验组35例早产儿通过视觉、触觉、听觉和前庭功能训练措施,进行早期干预,对照组常规儿科治疗护理,对两组新生儿分别于1、2、3、6、12月龄时进行生长激素(growth hormone,GH)和胰岛素样生长因子-1(insulin-like growth factor,IGF-1)检测。结果实验组GH在治疗后明显升高,3、6月时增长最快,6月达高峰,12月时结果与6月对比差异无统计学意义;IGF-1在实验组治疗后明显升高,6、12月时增加最快,12月时达高峰;对照组也在2、3、6月时出现增高,6月达高峰,但增长幅度明显小于实验组相应月份增长值。结论早期康复训练有利于早产儿发育。Objective To explore the impact of early rehabilitation training on the growth of premature infants with hypoxic ischemic encephalopathy. Methods Seventy premature infants, including 37 boys and 33 girls, were randomly divided into experimental group and control group (each n = 35). The infants of the two groups were similar in gender, mode of delivery, perinatal circumstances, parental age, educational background, occupational and economic situation, and living environment. There was no statistically significant difference between experimental group and control group in infants' gestational age (34.70 ±2.26 vs. 35.65± 1.96), birth weight (2.38±0.60 vs 2.68±0.82), and Apgar score (4.60±0.80 vs. 4.86±0.46). 35 infants of experimental group received early intervention in visual, tactile, auditory and vestibular function training, while the infants of control group were given routine pediatric treatment and nursing care. Growth hormone (GH) and insulin- like growth factor- 1 (IGF- 1) were detected in all infants at the ages of 1, 2, 3, 6, and 12 months, respectively. Results After the treatment, the GH level of experimental group was significantly elevated at the third and sixth months and it reached a peak at the sixth month, but no statistically significant difference was found in the GH level between the sixth and twelfth months. After the treatment, the IGF- 1 level of experimental group was significantly elevated at the sixth and twelfth months and it reached a peak at the twelfth month. The IGF- 1 level of control group was also increased at the second, third and sixth months and it reached a peak at the sixth month, but the growth values of control group were all significantly lower than those of experimental group in the corresponding months. Conclusion Early rehabilitation training is conducive to the growth of premature infants.
