六味地黄丸联合西药治疗高血压病的Meta分析和系统评价

摘　　要：目的对六味地黄丸联合西药治疗高血压患者的文献进行Meta分析,了解联合用药治疗高血压患者的疗效。方法全面检索中国期刊全文、中国生物医学、重庆维普、万方科技期刊数据库,查找六味地黄丸联合西药治疗高血压病的文献。采用Jadad评分量表对纳入文献进行质量评价,使用Revman5.0软件对临床疗效,治疗前、后的收缩压和舒张压进行Meta分析。结果经筛选后纳入14篇随机对照临床试验(RCT)文献,均是Jadad评分小于3分的低质量研究。Meta分析结果显示临床疗效的合并效应量RR为1.16,95%置信区间(95%CI)为(1.11,1.21);治疗前收缩压的合并效应量加权均数差WMD为3.14 mmHg,95%CI(0.44,5.83)mmHg,治疗后收缩压的合并效应量WMD为-8.29 mmHg,95%CI(-10.68,-5.91)mmHg;治疗前舒张压的合并效应量WMD为-1.03 mmHg,95%CI(-2.12,0.07)mmHg,治疗后舒张压的合并效应量WMD为-7.86 mmHg,95%CI(-11.03,-4.69)mmHg。结论六味地黄丸联合西药治疗高血压患者的总体疗效优于单纯西药组,治疗后的收缩压和舒张压也优于单纯西药组;六味地黄丸有助于提高高血压患者的总体疗效,降低患者的收缩压和舒张压。但是由于文献的质量不高,仍需要大样本双盲随机试验的理论支持。Objective： To understand the efficacy of combined medication on hypertension by using Meta-analysis and systematic reviews of Liuweidihuang Pills combined with western medicine to treat hypertension.Methods： Randomized controlled trials（RCT） papers were searched in China Academic Journal,CNKI,Chongqing VIP and Wanfang database.Jadad scale was used to devalue the quality of the included papers.Meta-analysis about clinical efficacy,systolic and diastolic blood pressure before and after treatment were analyzed in Revman 5.0 software.Results： 14 RCT papers were included after screening.They were low-quality papers with Jadad score less than 3 points.Meta-analysis showed that clinical efficacy of the combined effect RR was 1.16,95% CI（1.11,1.21）.Weighted mean difference（WMD） of Systolic blood pressure before treatment were-3.14mmHg,95%CI（0.44,5.83）mmHg,and systolic blood pressure after treatment were-8.29mmHg,95%CI（-10.68,-5.91）mmHg.Weighted mean difference（WMD） of diastolic blood pressure before treatment were-1.03mmHg,95% CI（-2.12,0.07）mmHg,and diastolic blood pressure after treatment were-7.86 mmHg,95% CI（-11.03,-4.69）mmHg.Conclusion： Liuweidihuang Pills can help to improve the overall efficacy of hypertension patients,reduce the patient＇s systolic blood pressure and diastolic blood pressure.However,due to the low quality of the literatures,large sample double-blind RCT should be carried out to support it.
