SIEMENS ADVIA 2120全自动血细胞分析仪LUC与幼稚细胞相关性分析

摘　　要：目的:探讨SIEMENS ADVIA 2120全自动血细胞分析仪分类计数LUC(large unstained cell,大的未染色细胞)与手工镜检分类计数幼稚细胞的相关性,从而明确LUC的临床应用价值。方法:应用ADVIA 2120全自动血细胞分析仪分别对100例健康体检者及300例诊断明确白血病患者全血标本分类计数LUC,同时手工镜检分类计数幼稚细胞,以手工镜检为参考标准,对两者的结果进行比较分析。结果:100例健康体检者全血标本应用血细胞分析仪分类计数的假阳性率(误诊率)是4%,真阴性率(特异度)是96%,假阴性率(漏诊率)是0%。300例白血病住院患者全血标本应用血细胞分析仪分类计数的真阳性率(灵敏度)是77.8%,假阳性率(误诊率)是32.2%,真阴性率(特异度)是67.8%,假阴性率(漏诊率)是22.2%。LUC与手工镜检分类计数幼稚细胞结果比较,相关性不好,差异有统计学意义(P<0.05)。结论:应用ADVIA 2120全自动血细胞分析仪对健康者分类计数LUC可作为一种过筛手段,但还不能完全替代手工计数,当仪器计数结果提示有LUC%异常时,一定要做手工镜检核实。应用ADVIA 2120全自动血细胞分析仪对白血病患者分类计数LUC时,每份全血标本均需要做手工镜检核实。Objective To study the relation of results of SIEMENS ADVIA 2120 automatic blood cells analyzer classification counting LUC and manual microscopy classification counting immature cells.Methods Whole blood specimen of 100 cases of the healthy check-up and 300 cases of the leukemia hospitalized patient, respectively,were detected by ADVIA2120 automatic blood cells analyzer and manual microscopy.For ref- erence standard with manual microscopy,the results of both were compared and analysed .Results The false-positive rate of whole blood speci- men of 100 cases the healthy check-up which were classification counted with blood ceils analyzer was 4% ,the really negative rate（specifici- ty）was 96%,the false-negative rate was 0%. The really positive rate（sensitivity）of whole blood specimen of 300 cases the leukemia hospital- ized patient which were classification counted with blood cells analyzer was 77.8% ,the false-positive rate was 32.2% ,the really negative rate （specificity）was 67.8%,the false-negative rate was 22.2%. The correlation of the leukemia patient＇s results of blood cells analyzer classifica- tion counting LUC and manual microscopy classification counting immature cells was bad,difference was statistically significant （P〈0.05）o When blood cells analyzer and manual microscopy two classification counting methods were under the condition of the positive, the correlation was bad, difference was statistically significant （P〈0.05）o Conclusion ADVIA2120 automatic blood ceils analyzer can be applied to classifi- cation count LUC of the healthy check-up as a kind of sifting counting method,but still do not replace manual counting. When instrument count suggested a LUC% anomalies,it must do manual microscopy.When ADVIA2120 automatic blood ceils analyzer was applied to classifica- tion count LUC of the leukemia patient.each soecimen all need to do manual microscopy to verify.
