前庭阵发症的影像学特点

摘　　要：目的探讨前庭阵发症(VP)的影像学特点。方法应用MRI三维稳态进动快速成像序列(3D-FIESTA)对51例VP患者(VP组)及51例其他眩晕病患者(对照组)的桥小脑角区进行轴位扫描,并比较其结果。结果 VP组42例(82.4%)患者存在神经血管交互压迫(NVCC),其中单侧37例(88.1%),双侧5例(11.9%);对照组8例(15.7%)患者有NVCC,均为单侧。VP组NVCC阳性率显著高于对照组(χ2=45.351,P<0.001)。VP组NVCCⅠ型23耳(48.9%),Ⅱ型5耳(10.6%),Ⅲ型17耳(36.2%),Ⅳ型2耳(4.3%);压迫小脑前下动脉45耳(95.7%)。对照组Ⅰ型4耳(50.0%),Ⅱ型1耳(12.5%),Ⅲ型3耳(37.5%),无Ⅳ型;压迫血管均为小脑前下动脉。VP组与对照组分型构成差异无统计学意义(χ2=0.367,P=0.947)。结论 VP患者NVCC的发生率较高,最常见的为单侧NVCCⅠ、Ⅲ型的前庭蜗神经与小脑前下动脉交互压迫。Objective To explore the imaging characters of vestibular paroxysmia （VP）. Methods The cerebellopontine angle area were axial scaued by MRI with three-dimensional last imaging employing steady state acquisition in 51 VP patients （VP group） and 51 other vertiginous patients （control group）, and the results were compared. Results Forty-two patients （82. 4% ） in VP group existed nerve vessel interactive compression （NVCC） , and unilateral in 37 cases （88.1% ） , bilateral in 5 cases （ 11.9% ）. Eight patients （ 15.7% ） in control group existed NVCC, and all of them were unilateral. The NVCC positive rate in VP group was significantly higher than control group （X^2 =45. 351, P 〈0. 001 ）. Twenty-three ears （48.9%） in VP group was NVCC type Ⅰ , 5 ears （ 10.6% ） was type Ⅱ , 17 ears （36.2%） was type Ⅲ and 2 ears （4.3%） was type Ⅳ ; anterior inferior cerebellar artery oppressed was in 45 ears （95.7%）. Four ears （50. 0% ） in control group was NVCC type Ⅰ , type Ⅱ in 1 ear （12.5%）, type Ⅲ in 3 ears （37.5%）, and there was no type Ⅳ; anterior inferior cerebellar artery was the only oppressed vessel. There was no statistical significance of NVCC types between VP group and control group （X^2 = 0. 367, P = 0. 947 ）. Conclusions The incidence of NVCC in VP patients is higher. The most common are unilateral NVCC types Ⅰ and Ⅲ involved vestibulocochlear nerve and cerebellar interactive compression.
