摘 要:目的探讨腰椎骨质增生与椎间盘膨出或突出的相关性,分析骨赘常见MRI信号类型。资料与方法回顾性分析行腰椎MRI检查的202例患者的影像学资料,分别记录腰椎有无骨赘形成、最长骨赘长度及信号、椎间盘膨出或突出程度,并分析所有患者及男性患者、女性患者各自最长骨赘长度与椎间盘膨出或突出程度之间的相关性。结果 202例腰椎图像包括1010个椎体,其中156个椎体有肉眼可见骨赘形成,MRI上骨赘主要表现为T1和T2等或高信号,STIR低信号,平均最长骨赘长度为(5.80±2.40)mm,平均椎间盘膨出或突出程度为(6.17±2.64)mm,两者呈显著正相关(r=0.83,P<0.05)。男性患者与女性患者平均最长骨赘长度分别为(6.61±2.49)mm和(5.06±2.06)mm,平均椎间盘膨出或突出程度分别为(7.28±2.80)mm和(5.13±2.00)mm,两者差异均有统计学意义(P<0.05),男性和女性患者最长骨赘长度与椎间盘膨出或突出程度均呈显著正相关(r=0.79、0.83,P<0.05)。结论腰椎骨质增生与椎间盘膨出或突出间有很强的相关性;T1和T2等或高信号,STIR低信号为MRI上骨赘的主要信号类型。Purpose To investigate the correlation between lumbar hyperostogeny and disc bulge or herniation, and to analyze the main MRI signals of osteophyte. Materials and Methods 202 patients underwent MRI of the lumbar spine. Osteophyte tbrmation, the maximum length and signal of osteophyte, degree of disc bulge or herniation of all patients were recorded. The correlation between the maximum length of osteophyte and degree of disc bulge or herniation of all patients, males and females were analyzed, respectively. Results 202 patients included 1010 vertebral bodies, 156 vertebral bodies involved macroscopic osteophyte. Osteophyte showed equal or high signal on TI and T2, low signal on STIR. The mean maximum osteophyte length was (5.80±2.40) mm, which was positively correlated with the mean disc bulge or herniation degree of (6.17±2.64) mm (r=0.83, P〈0.05). The mean maximum osteophyte length of males and females were (6.61 ±2.49) mm and (5.06±2.06) mm, respectively; the mean disc bulge or herniation degree were (7.28±2.80) mm and (5.13 ±2.00) mm, respectively, with statistical difference in each mean between males and females (P〈0.05). There were statistically positive correlations between maximum osteophyte length and disc bulge or herniation degree for males and females (t=0.79, 0.83, P〈0.05). Conclusion Lumbar hyperostosis is obviously correlated with disc bulge or herniation, equal or high signal on TI and T2, and low signal on STIR is the main signal type ofosteophyte.
