摘 要:目的探讨多层螺旋CT三维后处理技术在茎突测量中的应用及对茎突综合征(SPS)诊断的价值。方法使用64层螺旋CT的容积再现(VR)和多层面重建(MPR)技术对15例正常成人茎突和27例临床确诊为茎突综合征的茎突进行测量,测量茎突的长度、方位角及茎突远段至扁桃体外缘的最短距离,使用SPSS17.0软件进行统计学分析。结果病例组茎突长度平均值为(3.52±1.04)cm,向内偏斜角为24.89°±5.58°,向前偏斜角为8.37°±3.88°,茎突与扁桃体间距为(0.65±0.48)cm,茎突长度、向内偏斜角及茎突与扁桃体间距在正常组与病例组间有显著统计学意义(P〈0.01),向前偏斜角在正常组与病例组间无显著差异;茎突与扁桃体间距与茎突长度及向内偏斜角间存在线性回归关系(P〈0.01)。结论茎突过长、方位角异常是诊断SPS的重要依据,茎突与扁桃体间距缩短可作为补充依据,多层螺旋CT三维后处理技术可为诊断SPS提供依据。Objective To explore the application of multi-slice spiral CT three-dimensional post-processing technique in the styloid process measurement and its diagnostic value of the styloid process sydrome (SPS). Methods The styloids process of 15 normal adults and 27 SPS patients were scanned through 64-slice CT volume rendering(VR) and reconstructed through multi-planar recon- struction(MPR) technique to measure the length,azimuth and the shortest distance from the outer edge of tonsil to the distal styloid process. The results were analyzed by SPSS 17.0. Results The styloid process average length of the patients group was ( 3.52 ±1.04) cm, the average inward deflection angle was 24.89°± 5.58°, the average forward deflection angle was 8.37°±3.88° and the average shortest distance from the outer edge of tonsil to the distal styloid process was (0. 65 ± 0. 48) cm. The styloid process length, the inward deflection angle and the shortest distance from the outer edge of tonsil to the distal styloid process were signifi- cantly different from the normal adults group (P〈0.01). However ,the differences of the forward deflection angle between the nor- mal and the patients group were no significant. Meanwhile there are significant linear relationships between the shortest distance from the outer edge of tonsil to the distal styloid process and the styloid process length, the shortest distance from the outer edge of tonsil to the distal styloid process and the inward deflection angle (P〈0. 01). Conclusion Over-length and azimuth anormaly of the styloid process can provide an important basis for diagnosis of SPS, and the shorten distance between the styloid process and the ton sils can be used as complementary basis. In sum multi-slice spiral CT three-dimensional post-processing technique can provide basis on the diagnosis of SPS.
