摘　　要：目的探讨Castleman病的CT表现及其诊断价值。方法回顾性分析17例经病理证实的Castleman病的临床资料和CT表现，并与病理结果进行对比分析。结果病变部位：胸部7例，腹膜后6例，肠系膜2例，颈部2例。临床分型：局灶型12例，多中心型5例。病理分型：透明血管型13例，浆细胞型4例。CT表现：CT平扫示病灶均为等密度，3例伴有钙化；增强扫描示透明血管型病灶均有明显持续强化，浆细胞型病灶呈轻度一中度强化。结论Castleman病的CT表现与病理类型密切相关，透明血管型病变的CT表现具有特征性。Objective To study the CT findings of Castleman＇s disease and the diagnostic value of CT for Castleman＇s disease. Methods Clinical data and CT findings of Castleman＇s disease proved by pathology in 17 cases were retrospectively analyzed and cor- related with pathology. Results Locations of the lesions were as follows, thorax （ n= 7 ）, retroperitoneum （ n = 6 ）, mesentery （ n= 2 ） and neck（n= 2）. The clinical subtypes included localized type in 12 cases and multicentric type in 5 cases. As for histopathologic clas- sification, there were hyaline-vascular type in 13 cases and plasma cell type in 4 cases. Apart from 3 cases with intra-tumoral calcifica- tions,the other ones showed homogeneous attenuation on plain CT. After contrast administration, marked and sustained enhancement was showed in hyaline vascular type,mild-moderate enhancement was showed in plasma cell type. Conclusion CT findings of Castle- man＇s disease were closely correlated with pathological subtypes. CT features of hyaline-vascular subtype are characteristic.
