连续摄片法在儿童呼吸道异物诊断中的应用价值

摘　　要：目的:探讨连续摄片法在儿童呼吸道异物诊断中的应用价值。方法:搜集经纤维支气管镜证实的呼吸道异物患儿31例,以3岁为界分为2组,<3岁患儿组行连续摄片法和透视法(n=18,Ⅰ组),≥3岁患儿组行连续摄片法和吸呼气相胸片法(n=13,Ⅱ组),对每组中2种不同检查方法的诊断符合率、辐射量进行对比分析。结果:<3岁患儿组:连续摄片法图像质量较好,诊断符合率约为88.89%(16/18),辐射量为(0.25±0.04)mSv;透视法诊断符合率约为77.78%(14/18),辐射量为(1.24±0.09)mSv,二者辐射量的差异有统计学意义(P<0.05)。≥3岁患儿组:连续摄片法和吸呼气相胸片法诊断符合率均为84.62%(11/13),辐射量分别为(0.22±0.03)和(0.06±0.01)mSv,且二者差异有统计学意义(P<0.05)。结论:连续摄片法在<3岁儿童呼吸道异物诊断中具有重要价值。Objective：To explore the application of continuous radiography in diagnosing airway foreign body in children. Methods：Thirty-one eases of airway foreign body in children confirmed by fiberoptic-bronchoscopy were divided into two groups by the bound of 3 years of age. Children who were under 3 years of age were examined by continuous radiography and fluoroscopy （n=18） ,The others who were 3 years old and over three years of age were examined by continuous radiography and chest radiograph in deep inspiratory and expiratory phase （n= 13）. The diagnosis coincidence rate and radiation dose of two different methods in each group were compared and analyzed. Results： In group I , children who were ex amined by continuous radiography,image quality was good. The diagnosis coincidence rate was 88.89 ~ （16/18） and the ra- diation dose was （0.25士0.04）mSv. The diagnosis coincidence rate of fluoroscopy was 77.78~/00 （14/18） and the radiation dose was （1.24士0.09）mSv. The difference of radiation dose of two methods had statistical significance （P〈0.05）. In group II ,the diagnosis coincidence rates of two methods were both 84.62% （11/13）. The radiation doses of two methods were （0.22士0.03）mSv and （0.06士0.01）mSv respectively and the differences had statistical significance （P〈0.05）. Conclusion： Continuous radiography has important value in diagnosing airway foreign body of children when the children are less than 3 years old.
