摘 要:目的 :研究苍附导痰丸加减对多囊卵巢综合征(PCOS)大鼠肿瘤坏死因子-α(TNF-α)及其受体表达的影响,探讨苍附导痰丸加减治疗PCOS的机制。方法:雌性大鼠随机分成5组,每组大鼠10只。模型组:采用丙酸睾酮联合绒毛膜促性腺激素(HCG)建立PCOS大鼠病理模型;中药高剂量组:造模结束后24h用苍附导痰丸加减方24g/kg灌胃;中药中剂量组:造模结束后24h用苍附导痰丸加减方12g/kg灌胃;中药低剂量组:造模结束后24h用苍附导痰丸加减方6g/kg灌胃;克罗米芬组:造模结束后,第1~14天灌胃给予克罗米芬悬液;空白对照组:未造模大鼠颈背部皮下注射等量的生理盐水及继予灌胃等量生理盐水。14天后光镜观察卵巢大体形态变化,卵泡发育情况,放射免疫法测定血清雄激素(T)、雌二醇(E2)、促黄体生成素(LH),TNF-α,免疫组织化学法测定卵巢组织TNF-α、TNFR1、TNFR2表达。结果 :中药各组大鼠用药后均体重减轻,以中药高剂量组最为明显,与空白对照组比较无差异(P>0.05)。中药中、高剂量组及克罗米芬组大鼠的激素及TNF-α水平均显著改善(P<0.05),以中药高浓度组最为明显;中药高剂量组TNF-α、TNFR1、TNFR2蛋白表达显著减少(P<0.05),克罗米芬组TNF-α、TNFR2蛋白表达减少(P<0.05)。结论:苍附导痰丸加减对丙酸睾酮联合HCG建立的PCOS大鼠有一定的治疗作用,其机制可能是通过调节TNF-α、TNFR1、TNFR2蛋白表达,从而抑制了炎症信号的通路。Objective: To investigate the effect of Chinese medicine Modified Canffu Daotan Wan(MCDW) on the expression of tumor necrosis factor alpha(TNF-α) and its receptor in rat model of polycystic ovary syndrome(PCOS), and to further ex- plore its molecular therapeutic mechanism. Methods: Fifty female rats were randomized into 5 groups, blank control group, model group, positive control group, and high-, moderate- and low-dosage MCDW groups. Rats except for the blank con- trol group were given the subcutaneous injection of testosterone propionate and human chorionic gonadotropin(HCG) for in- ducing PCOS. From day 1 to 14 after the modeling, positive control group was given intragastric administration of clomiphene suspension, high-, moderate- and low-dosage MCDW groups were given CDW at the dosage of 24, 12 and 6g/kg 12 hours after the modeling, and the blank control group was given subcutaneous injection of same volume of normal saline at the nape and then given intragastric administration of same volume of normal saline. After treatment for 14 days, general morphology of rat ovary and follicular development were observed under light microscopy. Serum levels of testosterone(T), estradiol(E2), luteinizing hormone(LH) were determined by radioimmunoassay. Expression levels of TNF-α, TNF receptor-1 (TNFR1), and TNF receptor-2(TNFR2) in ovarial tissues were determined by immunohistochemical method. Results: Body weight was decreased in MCDW groups, and the decrease was obvious in middle-dosage MCDW group(P〉 0.05 comparedwith the blank control group). Serum hormone and TNF-α levels were improved in middle- and high-dosage CDW groups and positve control group, and the improvement was obvious in high-dosage MCDW group(P〈 0.05). High-dosage MCDW decreased TNF-α, TNFR1 and TNFR2 protein expression level, and clomiphene decreased TNF-α and TNFR2 protein ex- pression level(P〈 0.05). Conclusion.
