卡前列素氨丁三醇联合改良式补丁缝合术在前置胎盘产后出血中的应用

摘　　要：目的观察卡前列素氨丁三醇（欣母沛）联合改良式补丁缝合术在前置胎盘产后出血中的疗效。方法将62例前置胎盘患者随机分为对照组（30例）和治疗组（32例）。对照组采用欣母沛，治疗组采用欣母沛联合改良式补丁缝合术预防产后出血。比较两组术中、产后2h、产后24h出血量及血红蛋白改变情况。结果治疗组较对照组出血量术中[（321．9±67．8）ml与（435．9±83．3）ml]及产后2h[（417．7±68．0）ml与（539．64-70．4）ml]、24h[（485．3±68．5）ml与（621．0±81．3）ml]均减少（t=5．926、6．938、7．133，均P〈0．05）；治疗组血红蛋白产前产后无明显下降，血红蛋白变化较对照组[（5．0±6．5）g／L与（-4．0±6．8）g／L]差异有统计学意义（t=-3．662，P〈0．05）。结论欣母沛联合改良式补丁缝合术能更有效地预防前置胎盘产后出血。Objective To observe the effect of carboprost tromethamine（hemabate） combined with modified patch suture in placenta previa postpartum hemorrhage. Methods 62 patients with placenta previa were divided into the control group （ 3 0 cases ） and treatment group （ 3 2 cases ） . The control group was treated with hemabate, and the treatment group was treated with hemabate combined with modified patch suture. The two groups of intraoperative, postpartum 2 hours,24 hours postpartum were compared in bleeding volume and hemoglobin changes. Results The intraoperative bleeding volume [ （321.9 ± 67.8）ml vs （435.9 ± 83.3 ）ml] and 2h,24h postpartum bleeding volume [ （417.7 ±68.0）ml vs （539.6±70.4）ml and （485.3 ±68.5） ml vs （621.0 ± 81.3） ml]of the treatment group were significant less than that of the control group（t =5. 926,6. 938,7. 133 ,all P 〈0.05）. Significant difference in changes of hemoglobin level was also observed between the two groups, though postpartum level was not significantly reduced in the treatment group compared with the control group [ （ 5.0 ±6.5 ） g/L vs （ - 4.0 ± 6.8 ） g/L ] （ t = - 3. 662, P 〈 0. 05 ）. Conclusion Hemabate combined with modified patch suture can be more effective in the pre- vention of postpartum haemorrhage in placenta previa.
