临床路径对单病种住院天数和费用影响的研究

摘　　要：目的分析临床路径对住院日和住院费用的影响。方法收集2010.7-2012.3期间实施临床路径后诊断标准和治疗方案一致的4个病种住院患者纳入本研究,对比临床路径组和对照组住院日及住院费用。结果 1.临床路径降低了腮腺混合瘤患者的住院天数(P<0.05),其他病种无统计学差异。2.住院费用主要构成因子手术费、药费路径组低于对照组(P<0.05)。3.唇裂、腭裂患者的住院总费用路径组低于对照组(P<0.05)。结论本次研究的4个单病种通过实施临床路径减少了住院日、避免了平均药品费用的快速增长、有效减少了患者的住院总费用。Objective To analyze the influence of clinical pathways on the length of hospital stay and medical expenditure. Meth- otis Collecting all the cases of 4 single diseases that were consistant in diagnosis standard and treatment after the implementation of clinical pathways, then compared the trail and control group with the total length of stay as well as hospital expenditure. Results 1. The length of stay from the trail group is lower than it from the control group in the mixed parotid tumor, the difference is statistical- ly significant （ P 〈 0.05）. 2. The factors which make up the expenditure including drug expense, surgical fees in trail group is lower than the control group （ P 〈 0.05）. 3. The average hospital expenditure of the control group is higher than the trail group in the cleft lip and cleft palate, the result has statistically significant （P 〈 0.05）. Conclusion With the implementation of clinical pathway, the average stay length of four single diseases decreased and prevented the increasing of the average medical drugs cost, thus cut down the medical fee of the patients effectively.
