摘 要:目的研究出诊医生参与院外急救电话指导对急救救治效果的影响。方法选取本中心2010年5月~2011年5月间66895例院外急救病例,分为A、B两组,A组由未开展电话医学指导救治的35926例作为对照组。B组由出诊医生参与院外急救电话指导的30969例作为研究组。比较和分析两组间救治效果的差异。结果电话指导组救治有效率70.99%,较未开展电话医学指导组救治有效率62.86%,有显著的提高(X2=513.31,P〈0.01)。且放空率明显下降,服务满意率明显提高。结论出诊医生参与院外急救电话指导能提高救治效果,也能减少急救资源浪费和提高服务满意率,但值得注意的是电话指导语言必须通俗易懂、方案必须清晰、易于遵从。Objective To study the effects of participation in telephone guidance by emergency doctors. Methods 66 895 patients undergoing pre-hospital emergency treatment were randomly divided into 2 sex, age, type of disease, and condition-matched groups, Group A (n=35 926, not receiving the guidance of the pre-hospital emergency team) and Group B (n=30 969, receiving the guidance from the pre-hospital emergency team through phone call).The effective rate of pre-hospital emergency treatment, satisfaction rate of the patient and/or their families, and the empty-carrying rate of ambulance were analyzed and compared between these 2 groups. Results The empty carrying rate of Group B was 12.82 %, significantly lower than that of Group A (18.20 %, X2=379.34, P 〈 0.01). The satisfaction rate of the patients and/or their families of Group B was 100%, significantly higher than that of Group A (92.80, X 2=35.63, P 〈 0.01). The effective rate of pre-hospital treatment of Group B was 70.99%, significantly higher'than that of Group A (62.87 %, X2=513.37, P 〈 0.01). Conclusion Emergency doctor participating in first aid telephone guidance increases the survival rate of the patients, reduces the waste of resources, and improve the satisfaction rate of the patients and/or their families . The language of telephone guidance must be easy to understand and follow.
