加压冷疗系统在前交叉韧带术后康复训练的疗效观察

摘　　要：目的探讨加压冷疗系统对前交叉韧带重建术后康复训练效果的影响。方法将120例行膝关节前交叉韧带重建术患者按随机数字表法分为试验组（加压冷疗系统康复）和对照组（传统康复），每组60例。两组患者术后系统康复训练3个月，并随访半年。观察并比较两组患者术后疼痛评分、肿胀值、Lysholm及IKDC评分。结果试验组术后6、24、48h疼痛视觉模拟量表（VAS）评分较对照组明显减轻，差异有统计学意义（P〈0．01）。两组患者术后6h、12h、24h、48h、1周、2周肿胀值比较差异有统计学意义（P〈0．05）。试验组患者术后4周、3个月、6个月肿胀值小于对照组，但差异无统计学意义（P〉0．05）。术后4周Lysholm、IKDC评分试验组分别为（65．2±12．3）、（65．3±14．3）分，对照组分别为（60．2±12．5）、（59．4±13．5）分，两组比较差异有统计学意义（P〈0．05）。术后3、6个月Lysholm、IKDC评分两组比较差异无统计学意义（P〉0．05）。结论加压冷疗系统可以减轻前交叉韧带重建术后早期的疼痛和肿胀，提高早期康复训练的效果，但对长期的康复训练效果影响不大。Objective To explore the effect of cryotherapy and compression system in rehabilitation treatment for reconstruction of anterior crueiate ligament. Methods One hundred and twenty patients who had been treated with reconstruction of anterior cruciate ligament were randomly divided into treatment group （60 patients, treated with cryotherapy and compression system） and control group （60 patients, treated with traditional method ）. All patients was treated for 3 months and followed up for 6 months. The scores of visual analogue scales（VAS）, degree of swelling, Lysholm and IKDC scores were observed and compared. Results The scores of VAS in treatment group postoperative 6,24 and 48 h were significantly lower than those in control group （P〈 0.01 ）. The degree of swelling in two groups postoperative 6 h, 12 h,24 h,48 h, 1 week,2 weeks had significant difference（P 〈 0.05）. The degree of swelling in treatment group postoperative 4 weeks, 3 months and 6 months was lower than that in control group,but there was no significant difference （P 〉 0.05）. The scores of Lysholm and IKDC in treatment group postoperative 4 weeks were （65.2 ± 12.3）, （65.3 ± 14.3） scores,in control group were （60.2± 12.5）, （59.4 ± 13.5） scores,there was significant difference （P 〈 0.05）. The scores of Lysholm and IKDC in two groups postoperative 3,6 months had no significant difference （P 〉 0.05）. Conclusion The cryotherapy and compression system in rehabilitation treatment for reconstruction of anterior cruciate ligament can improve the effect of early-term rehabilitation training, but has little impact on the long-term rehabilitation training.
