小剂量阿托伐他汀联合阿司匹林对老年患者颈动脉粥样硬化斑块及血清炎症水平影响的研究

摘　　要：目的探讨临床小剂量阿托伐他汀联合阿司匹林对老年患者颈动脉粥样硬化斑块及血清炎症水平的影响。方法选择上海交通大学医学院附属新华医院2010年4月～2011年5月经B超证实存在颈动脉粥样硬化斑块的老年患者105例,将其分为两组。小剂量阿托伐他汀联合阿司匹林联合治疗组(治疗组,55例)给予阿托伐他汀20mg/d和阿司匹林100mg/d;阿司匹林单独治疗组(对照组,50例)仅给予阿司匹林100mg/d。疗程6个月。分别在治疗前、后测定血脂、颈动脉内膜中层厚度(IMT)、颈动脉斑块积分以及血清高敏C反应蛋白(hs-CRP)水平。结果治疗组治疗后血脂指标、IMT、颈动脉斑块积分以及血清hs-CRP水平比治疗前显著下降(均P<0.01),且明显优于对照组,差异均有高度统计学意义(均P<0.01)。结论小剂量阿托伐他汀联合阿司匹林治疗可改善老年患者颈动脉粥样硬化程度及炎症反应程度,具有稳定斑块作用,且不增加出血发生率。Objective To investigate the effect of low-dosage Atorvastatin plus Asprin on carotid atherosclerosis plaque and inflammation in elderly patients. Methods 105 elderly subjects with B-mode ultrasound-proven carotid atherosclerosis in Xinhua hospital affiliated to Shanghai jiaotong university school of medicine from April 2010 to May 2011 were randomly assigned into two groups. Low-dosage Atorvastatin plus Asprin group （treatment group, 55 cases） was given Atorvastatin 20 mg/d and Asprin 100 mg/d; only received Asprin group （control group, 50 cases） was given only Asprin 100 mg/d. The course was 6 months. The blood lipid, carotid intima-medial thickness （IMT）, accumulate score of carotid plaque and serum hs-CRP levels were assessed before and after treatment. Results In the treatment group, lipid profile, IMT, accumulate score of carotid plaque and plasma hs-CRP levels were all significantly reduced compared with before treatment （all P 〈 0.01）, and also were superior to control group, the differences were all statistically significant different （all P 〈 0.01）. Conclusion Application of low-dosage Atorvastatin plus Asprin can improve carotid arteriosclerosis and inflammatory reaction in elderly patients, can stabilize plaque and doesn＇t increase the in- cidence of hemorrhage.
