经直肠超声引导下前列腺穿刺活检术后出血护理相关因素分析

摘　　要：目的探讨经直肠B超引导下前列腺穿刺活检术后出血的护理措施。方法收集我院泌尿科行经直肠B超引导下前列腺穿刺活检276患者的临床护理资料,对患者的年龄、疾病性质、术前凝血酶原时间(PT)、是否服用抗凝药物、压迫止血材料、压迫止血时间、是否应用止血药等资料进行统计,采用单因素分析前列腺术后出血的护理相关因素,采用多因素Logistic回归分析经直肠超声引导下前列腺穿刺活检术后出血的护理风险因素。结果单因素分析结果显示,术前PT时间、是否服用抗凝药物、压迫止血材料、压迫止血时间是经直肠超声引导下前列腺穿刺活检术后出血的相关护理因素。Logistic回归分析显示:PT时间及局部压迫时间是经直肠超声引导下前列腺穿刺活检术后出血的独立影像因素。结论经直肠超声引导下前列腺穿刺活检术后出血与患者术前的凝血功能及术后局部压迫时间有关,护理过程要给予充分的重视及护理干预。Objective To investigate the nusing correlative factor and nursing measure of postoperative bleeding after prostate biopsy guided by tansrectal ultrasound. Methods 276 cases of clinical data that taken tansrectal ultrasound guided prostate biopsy in our hospital were cdllected, age, pathological nature, preoperative PT time, whether or not take anticoagulant, materials of oppression hemostasis, time of oppression hemostasis and whether or not take hemo- static were statistics, nusing correlative factor ofostoperative bleeding after tansrectal ultrasound guided prostate biopsy were analyzed with monofactorial test, nusing influencing factor of ostoperative bleeding after tansrectal ultrasound guided prostate biopsy were analyzed with muhifactorial Logistic regression analysis. Results Monofactorial test showed that preoperative PT time, whether or not take anticoagulant, materials of oppression hemostasis and time of oppression hemostasis were corrective factors of postoperative bleeding after prostate biopsy guided by tansrectal ultrasound. Multifactorial Logistic regression analysis showed preoperative PT time and time of oppression hemostasis were influencing factor of postoperative bleeding after prostate biopsy guided by tansrectal ultrasound. Conclusion Postoperative bleeding after prostate biopsy guided by tansrectal ultrasound is corrective with thromboxane function and time of oppression hemostasis, attention and nursing intervention should be taken during clinical nursing.
