摘　　要：目的探讨HLA-B27/B7抗原检测在诊断强直性脊柱炎(AS)中的临床价值。方法选取144例门诊及住院高度疑似AS的患者,应用流式细胞仪对其标本进行HLA-B27/B7抗原检测。结果HLA-B27抗原阳性率在青少年人群比较高,且男性显著高于女性,受检的144例患者,HLA-B27抗原阳性率为63.9%,低于确诊为AS的阳性率(91.1%)。HLA-B27抗原检测对诊断AS的灵敏度为91.1%(82/90),特异度为81.5%(44/54),正确率为87.5%(126/144)。结论 HLA-B27抗原与AS密切相关,对临床诊断AS很有帮助,用流式细胞术进行检测是较好的方法。Objective To discuss the significance of detection of HLA-B27/B7 in clinical diagnosis of ankylosing spondysis.Methods Flow cytometry was used to detect the HLA-B27/B7 antigen.Results The HLA-B27 positive rate of all patients was 63.9%,which is higher in youths and men,and it was lower than AS patients＇ positive rate（91.1%）.The sensitive of HLA-B27 antigen in diagnosis of AS was 91.1%（82/90）,the specificity was 81.5%（44/54）,and the accuracy was 87.5%（126/144）.Conclusion The HLA-B27 antigen is highly related to the AS,and helpful to the diagnosis of ankylosing spondysis.The flow cytometry is a good method to the detection of HLA-B27 antigen.
