残髓炎误诊误治分析

摘　　要：目的:分析了解残髓炎的致病因素,以便及时纠正临床操作中的失误,彻底治疗牙髓病和根尖周病。方法:对116例残髓炎患者进行分析,并从临床角度对残髓炎临床表现分析分类,采用根管治疗术、牙髓塑化术,并对治疗方法进行评估。结果:误诊为尖周炎100例(86.31%);误诊牙周膜炎16例(13.69%);误诊时间5天～5年;因误诊拔除42例(36.03%)。结论:残髓炎为牙髓炎的一种类型,症状不典型。自发痛常为隐痛、钝痛。患者多以冷刺激痛为主诉,但对冷刺激又不敏感。这种主诉与检查结果不甚相符的现象是残髓炎的特点之一。由于炎症可能累及根尖周,患牙常伴有咬合痛或咬合不适感,这是残髓炎的另一特点。残髓炎多为干髓术后的并发症。避免牙髓炎的最好方法是行完善的根管治疗。严格掌握干髓或变异干髓术适应证,以免残髓滞留导致残髓炎的发生。Objective：to analyze the residual pulpitis of pathogenic factors, so as to cor- rect the errors in the clinical operation, thor- ough treatment of pulpitis and periapical pe- riodontitis. Methods ： 116 cases diagnosed as residual pulpitis were analyzed, and from the clinical perspective of residual pulpitis clini- cal manifestation analysis by classification, root canal therapy, dental pulp resinifying therapy, and the treatment method for the assessment of. Results： misdiagnosed as periapieal lesion in 100 cases, accounting for 86. 31% ; Misdiagnosis of periodontal in- flammation in 16 cases, accounting for 13. 69% of the longest 5 years; Misdiagnosis; a minimum of 5 days, due to misdiagnosis re- moval in 42 cases, accounting for 36.03%. Conclusion： the remnant pulpitis for end- odontitis one kind of type, atypical symp- toms. Spontaneous pain often pain, dull pain. Patients with cold to stimulate the pain as chief complaint, but not sensitive to cold stimulation. The complaint with the inspec- tion result accord with phenomenon is one of the characteristics of residual pulpitis. As a result of inflammation may be involved in periapical teeth, often accompanied by oc- clusal pain or occlusal discomfort, which is another characteristic of residual pulpitis. Residual pulpitis for mummification postop- erative complications. The best method is to avoid pulpitis in root canal therapy. Strict control of stern pith or variable mummifica- tion indications, lest residual pulp retention has residual pulpitis occurrence.
