摘 要:目的观察"通利大肠"对慢性阻塞性肺疾病(COPD)模型大鼠外周血及肺泡灌洗液中白介素-8(IL-8)、白介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)和白介素-10(IL-10)含量变化,从炎症调控角度探讨COPD"从肠论治"的效应机制。方法用气管注射脂多糖加熏香烟联合造模方法建立COPD大鼠模型,随机分为正常组、模型组、治肠组、治肺组及肺肠同治组。正常组、模型组灌胃纯净水,各给药组灌胃相应中药,连续14 d。用酶联免疫法(ELISA)测定外周血和肺泡灌洗液中细胞因子IL-8、IL-1β、TNF-α和IL-10的含量。结果①与正常组比较,模型组大鼠外周血和肺泡灌洗液的IL-8、IL-1β、TNF-α和IL-10含量均升高(P<0.01或P<0.05)。②与模型组比较,治肠组外周血和肺泡灌洗液的IL-8、IL-1β、TNF-α和IL-10含量均降低(P<0.01或P<0.05)。③与治肺组比较,肺肠同治组外周血IL-1β含量和肺泡灌洗液IL-8含量降低(P<0.01或P<0.05)。结论通利大肠或在治肺的基础上增加通利大肠,能降低COPD模型大鼠的外周血和肺泡灌洗液中的细胞因子含量,从而减轻全身及局部的炎症反应,这可能是COPD"从肠论治"效应产生的作用环节之一。Objective To observe the influences of dredging large intestine on content changes of interleukin-8 ( IL-8 ), interleukin-1β ( IL-1β ), tumor necrosis factor-α ( TNF-α ) and interleukin-10 (IL-10) in peripheral blood and bronchoalveolar lavage fluid (BALF) in rats with chronic obstructive pulmonary disease ( COPD), and investigate the mechanism of treating COPD from intestine from the an- gle of inflammation controlling. Methods The rat model of COPD was established by using intratracheal injection of LPS combining cigarette smoking. All rats were randomly divided into normal group, model group, treating intestine group ( intestine group), treating lung group ( lung group) and treating lung and intestine group (lung-intestine group). Normal group and model group were intragastrically given purified water and other groups were given corresponding Chinese medicinals for 14 days. The content of IL-8, IL-1β, TNF-α and IL-10 in peripheral blood and BALF were detected by applying enzyme-linked immunosorbent assay (ELISA). Results ①Compared with normal group, the content of IL-8, IL-1 β, TNF-α and IL-10 in peripheral blood and BALF all increased in intestine group (P 〈 0.01 or P 〈 0.05 ). ②Compared with model group, the content of IL-8, IL-1β , TNF-α and IL-10 in peripheral blood andBALF decreased in intestine group (P 〈 0. 01 or P 〈 0. 05). ③Compared with lung group, the content of IL-1β in peripheral blood and IL-8 in BALF decreased in lung-intestine group (P 〈0.01 or P 〈0.05). Conclusion Dredging large intestine or dredging large intestine based on treating lung can reduce the content of cytokines in peripheral blood and BALF to relieve general or local inflammatory reaction in rat with COPD, which may be one of effects of treating COPD from intestine.
