喉罩和自制插管引导管用于气管切开患儿行气道异物取出术的麻醉处理

摘　　要：报道1例气管切开异物取出术后支气管内异物残留的患儿行支气管镜检和异物取出术的麻醉过程。术中采用自制的插管引导管（gum-elasticbougie，bougie）引导金属气管导管通过气管切开口以预防金属导管置人困难。在术中及术后发生缺氧而经面罩通气不佳时用喉罩盖住气管切开口行正压通气以改善缺氧症状。This case report described the anesthesia process of an airway foreign body removal with rigid bronchoscope for a child who already underwent tracheotomy. To prevent difficult intraoperative intubation, a metal tracheal tube was guided by selfmade gum-elastic bougie （bougie） through the tracheotomy sinus. The laryngeal mask was applied to perform positive pressure ventilation via the tracheotomy sinus for improving the perioperative hypoxia symptoms, when hypoxia and poor mask ventilation happened.
