慢性特发性荨麻疹患者外周血总IgE与C3,C4,IL-2,IL-4,IFN-γ的相关性

摘　　要：目的通过研究慢性特发性荨麻疹(CIU)患者外周血总IgE与C3,C4,IL-2,IL-4,IFN-γ的相关性,进一步探讨CIU患者外周血总IgE与Th1/Th2细胞失衡相关性。方法 147例CIU患者为实验组,以59例急性荨麻疹(AU)患者、38例健康人为对照。采用双抗体夹心ELISA法进行检测外周血总IgE水平,同时运用IMMAGE双光径免疫浊度分析仪检测外周血IgG,C3,C4水平。采用ELISA法进行同批检测IL-2,IL-4,IFN-γ水平。采用t检验和直线相关分析分析各组检测值。结果 CIU组总IgE水平显低于AU组,但AU与CIU组总IgE水平高于正常对照组(P<0.05)。CIU和AU组IL-2,IFN-γ均低于正常对照组(P<0.05)。CIU组IL-4水平均高于正常对照组(P<0.05),但CIU组与AU组IL-4水平差异无统计学意义(P>0.05)。CIU患者经治疗后第一次复诊及停药30d后第二次复诊无症状组总IgE,IL-4水平均低于治疗前(P<0.05)。第二次复诊有症状组各检测值与治疗前组差异均无统计学学意义(P>0.05)。CIU患者总IgE水平与IL-4水平呈正相关(P<0.05),与IL-2,IFN-γ水平呈负相关(P<0.05)。结论外周血总IgE水平升高可能与Th1/Th2细胞失衡相关,降低CIU患者体内总IgE水平,可能有利于恢复Th1/Th2细胞平衡。Objective To investigate chronic idopathic urticaria＇ s （CIU） total serum IgE relation to C3, C4, IL-2, IL- 4, and IFN-γ in peripheral blood, and further to investigate chronic urticaria＇ s total serum IgE relation to Thl/Th2 out of balance. Methods The total serum IgE had been measured by sandwich ELISA, the e pression of C3, C4 and IgG had been measured by Immune Turbidimetry and the expression of IL-2, IL-4, IFN-γhad been measured by ELISA in 147 patients with CIU, 59 patients with acute urticaria（AU）, 38 normal controls. All groups of date were analysised by t test and linear correlation. Results The total serum IgE were lower in patients with CIU than AU, but the total serum IgE were higher in CIU and AU patients than in the normal controls（ P 〈 0.05 ）. The expression of IL-2, IFN-γ were lower in patients with CIU and AU than in the normal controls between in patients with CIU and AU （ P 〈 0.05 ）. The expression of IL-4 were higher in patients with CIU and AU than in the normal controls （ P 〈 0.05 ）. The expression of total IgE and IL-4 were lower in CIU patients who first and second returned visit without symptom after treatment than before（ P 〈 0. 05 ）. There were no significant difference between second returned visit patients with symptom and before treatment（ P 〉 0.05 ）. The expression of total IgE in CIU patient＇ s was positive correlation to the level of IL-4 （ P 〈 0.05 ）, but was negative correlation to the level of IL-2, IFN-γ （ P 〈 0.05 ）. Conclusion The expression of serum total IgE and IL-4 might be relation to Thl/Th2 out of balance. If depressing the expression of serum total IgE, it might be favour of recovering the Thl/Th2 balance.
