摘  要:本研究用复聪汤对工业噪声致感音神经性耳聋动物进行治疗,观察其对听神经和耳蜗毛细胞的修复再生作用效果。方法:54只健康青年豚鼠听力学检查后,42只动物给予工业噪声(96-102dB SPL)10d暴露致聋,动物致聋后随机分成两组,噪声治疗组22只用纯中药制剂给予"复聪汤Ⅰ号"口服液和"复聪汤Ⅱ号"滴耳液治疗;噪声对照组20只给予同样方法口服0.9%氯化钠溶液,另有12只为正常对照组。在治疗30,60和90d后测定实验动物的听功能脑干诱发电位(ABR),畸变耳声发射(DPOAE)后,每组处死6只动物,耳蜗左侧扫描电镜观察,右侧耳蜗铺片行光镜观察和毛细胞计数。结果:实验前ABR和DPOAE测定所有受试豚鼠的听功能正常;在连续10d工业噪声暴露后,豚鼠ABR反应阈值上升到40到60 dBSPL,DPOAE幅值降低。电镜观察可见耳蜗毛细胞静纤毛折断、散乱和融合,以及中回毛细胞的损害消失;在用复聪汤口服和耳浴治疗30d后,80%的治疗组豚鼠ABR反应阈值恢复到30到40 dBSPL,DPOAE幅值有较明显提高;耳蜗铺片和电镜观察显示耳聋豚鼠的耳蜗毛细胞数目有一定恢复,组织结构有修复现象,电镜可见再生毛细胞仅出现少量纤毛,而未治疗噪声对照组未发现此种现象;治疗60d时,耳蜗毛细胞数目明显增多,Corti器的毛细胞区,有大量的增殖细胞出现,扫描电镜可见成小束的新生纤毛出现在毛细胞缺失部位,同时支持细胞大量增殖;到治疗90d时,再生的静纤毛束已基本形成,尤其是耳蜗基底部位的毛细胞明显增多,听功能已基本正常。结论:复聪汤对听力损失豚鼠耳蜗毛细胞有一定的修复和再生作用。Objective: The permanent sensorineural deafness is mainly caused by the damage or loss of the acoustic nerve and cochlea hair cells induced by noise, which is considered as inreversible by present. By studying of the effect of traditional Chinese medicine(TCM) in treating the damage of the hair cells loss induced by industrial noise, to observe the effect of the regeneration and recovery of the acoustic nerve and cochlear hair cells. Methods: 54 adult guinea pigs were used in this experiment. Auditory function test of auditory brainstem response(ABR) and distortion product otoacoustic emission(DPOAE) was performed on the animals. And 42 deaf animals induced by exposure to industrial noise for 10 days, 8 hours each day, were divided into two groups. Another 12 guinea pigs were used as normal control group. 22 deaf animals induced by exposure to noise were treated by Fucong Decoction of TCM for consecutive 30 days to 90 days. Another 20 deaf animals were treated by same volume of physiological saline. After 30days, 60days and 90days of treatment, the experiment animals were tested auditory function by ABR and DPOAE. 6 animals of each group were killed after auditory function test, the left sides of the cochlear surface were observed by light microscope, and the hair cells counting of the right sides of the cochlear morphological changes were observed by scanning electronic microscopes(SEM). Results: Before experiment, all animals had a normal ABR threshold and DPOAE amplitudes value. After 10 days of noise exposure, the ABR threshold were increased to 50-80 dB SPL, and DPOAE amplitudes value was decreased significantly, which indicated that the hair cells and acoustic nerve were damaged. Light and SEM showed that the stereocilium of the cochlear hair cells were broken, scattered and mixed together, and the damage of the middle hair cells disappeared.
