联合血液净化治疗儿童危重症合并急性肾衰的护理

摘　　要：目的:探讨危重症患儿合并急性肾衰(ARF)应用连续性血液净(CBP)化联合血液灌流(HP)治疗的护理效果。方法:采用CBP联合HP治疗儿童危重症合并ARF 15例,治疗过程中进行专人护理和严密的观察,并比较治疗前后生命体征、有创动力学监测中心静脉压(CVP)、平均动脉压(MAP)、血清肌酐(SCr)、尿素氮(BUN)等指标的变化,采用SPSS13.0软件包进行统计学处理,以P<0.05为差异有统计学意义。结果:治疗后患儿SCr、BUN明显降低,差异有统计学意义(P<0.05),治疗过程中血流动力学稳定,生命体征平稳。结论:严密的观察及认真、仔细、安全护理对危重症患儿合并急性肾衰的有效血液净化治疗十分重要。Objective： To explore the nursing effect of continuous blood purification（CBP） combined with hemop- effusion （HP） treatment on critically ill children with acute renal failure （ARF）.Methods：Fifteen critically ill children with ARF were treated with CBP combined with HP were studied. The specially-designated nurses and strict obser- vation were carried out in the treatment process. The changes urea nitrogen（BUN）,serum creatinine（Cr）, of indexes were observed including vital signs,blood central venous pressure（CVP）, mean arterial pressure（MAP）etc before and after the treatment.The statistical analysis of the results was performed using the SPSS statistical package version 13.0. Significance was taken as a p value less than O.05.Results：The level of blood BUN, SCr were significantly reduced af- ter CBP combined HP treatment,and the differences were statistically significant （P〈0.05）;the hemodynamics and vital signs of patients were kept stable during the treatment. Conclusion：Strict observation, careful and safe nursing during the blood purification treatment are of great significance to the critically ill children with ARF.
