摘  要:目的探讨髓过氧化物酶指数(MPXI)在缺铁性贫血中的诊断价值。方法采用西门子ADVIA2120全自动血液分析仪检测59名临床诊断为缺铁性贫血患者和52名健康对照者的髓过氧化物酶指数和血红蛋白浓度(HGB),同时用美国Beckman-Coulter Access化学发光分析仪测定血清铁蛋白(SF)质量浓度,将所得数据进行统计学分析。结果 IDA患者的MPXI为(-4.97±7.94)明显低于健康对照者的MPX(I0.58±3.21,P<0.01);而IDA患者的SF为(11.5±2.7)μg/L明显低于健康对照者的SF([54.5±15.6)μg/L,P<0.01];MPXI与同一标本的SF水平呈正相关,r值为0.738,P<0.01,差异有显著性;MPXI与同一标本的HGB水平呈正相关,r值为0.809,P<0.01,差异有显著性。根据制定的ROC曲线,MPXI在本研究中测定的最佳临界值为-2.374,曲线下面积为0.885,P均<0.01。结论 MPXI的变化在IDA诊断中有较高的应用价值。[ Objective ] To observe the clinical value and significance of the myelo-peroxidase index (MPXI) in diagnosis of iron deficiency anemia. [Methods] MPXI and HGB of the normal controls (52cases), iron deficiency anemia (59 cases), respectively, were detected by Siemens ADVIA2120i automatic blood cell analyzer. At the same time, SF was detected by Beckman-Coulter Access Chemiluminescent analyzer. [ Results ] MPXI in the patients of iron deficiency anemia were significantly lower (-4.97±7.94) compared to normal controls (0.58±3.21, P 〈0.01), SF in the patients of iron deficiency anemia was significantly lower (11.5±2.7)μg/L compared to normal controls (54.5+ 15.6)p^g/L, P 〈0.01, the MPX1 was positively correlated with the SF in the same patient (r =0.738, P 〈0.01), the MPX1 was positively correlated with the HGB in the same patient (r =0.809, P 〈0.01), based on the ROC curve, the cut-off values and AUC of MPXI were -2.374 and 0.885, respectively (P 〈0.01). [Conclusion] MPXI is an important indexe for diagnosis of iron deficiency anemia.
