尿毒症患者血管钙化的相关危险因素分析

摘　　要：目的了解我院血液透析中心血液透析患者的血管钙化情况及相关危险因素。方法纳入我院血液透析中心91例血液透析患者,记录相关人口学资料及临床资料,完善实验室检查。分别以X射线片(腰椎侧位、双手、骨盆)评价血管钙化情况并分析其相关危险因素。结果 91中有钙化69例,无钙化22例,钙化的69例中轻度钙化38例(41.76%)、中度钙化14例(19.72%)、重度钙化17例(23.94%);钙化评分≤3分(60例)与钙化评分>3分(31例)两组间的年龄、透析龄、血磷、血PTH和钙磷乘积差异有统计学意义;Logistic回归分析显示年龄、PTH为中重度血管钙化的危险因素。结论维持性血液透析患者血管钙化发病率高,年龄、透析龄、血磷、血PTH、钙磷乘积与钙化发生发展相关,年龄、PTH可能是中重度血管钙化的独立危险因素。Objective To study the status of vascular calcification （VC） and the related risk factors in patients with uremia. Methods Ninety-one maintenance hemodialysis （MHD） patients were enrolled. The general information of the patients was recorded, and serum levels of calcium, phosphorous and intact parathyroid hormone （iPTH） were measured. X-ray examinations of hands, pelvis and lateral lumbar spine were used to evaluate vascular calcification. These data were statistically analyzed. Results Sixty-nine patients had different degrees of vascular calcification, and 22 patients were free of vascular calcification. Of the VC 69 patients,38 suffered from mild （41.76%）, 14 moderate （ 19. 72% ） and 17 severe （ 23.94% ） vascular calcification. The age, dialysis period, iPTH, calcium-phos- phate product and phosphorus level were significantly higher in the patients with vascular calcification score 〉3 than those with score ≤ 3. The Logistic regression analysis showed that age and iPTH were the risk factors for the modemte and severe vascular calcification. Conclusion MHD patients have high incidences of the vascular calcifi- cation. The age, dialysis period, iPTH, calcium-phosphate product and phosphorus can affect vascular calcification. Age and iPTH are the risk factors of the moderate and severe vascular calcification.
