彩色多普勒超声在妇科肿瘤术后患者下肢深静脉血栓诊断中的应用

摘　　要：目的探讨彩色多普勒超声在妇科肿瘤术后患者下肢深静脉血栓诊断中的价值。方法对112例常规妇科肿瘤术后患者进行下肢静脉超声检查,观察下肢深静脉的形态、结构、内部回声、彩色血流图像及多普勒血流频谱。结果诊断为下肢静脉血栓者52例(血栓组),未发现血栓者60例(对照组);血栓组均为周围型血栓,左下肢静脉血栓发生率较右侧高,差异有统计学意义(P<0.05);腹部手术和腹腔镜手术后患者血栓形成的发生率差异无统计学意义;与对照组比较,血栓组患者股静脉、腘静脉管径明显增宽,血流速度减慢,差异有统计学意义(P<0.05);彩色多普勒超声检查较D-二聚体检测具有较高的敏感性和特异性,可作为下肢静脉血栓检查的有效方法。结论彩色多普勒超声可以准确地评估妇科肿瘤术后下肢静脉血栓患者,具有较高的临床应用价值。Objective To investigate the diagnostic value of color Doppler ultrasound in the diagnosis of patients with deep venous thrombosis after gynecologic oncology.Methods 112 cases of routine postoperative gynecologic oncology patients were observed by using ultrasound for the deep vein of form,structure,internal echo,color flow images and Doppler blood flow spectrum. Results There were 52 cases diagnosed of deep vein thrombosis（thrombosis group）,and 60 cases were not found thrombosis（control group）;52 cases of thrombosis group were peripheral thrombosis, and the incidence of thrombosis in the left legs was higher than in the right legs.They were significantly different（P 0.05）.Compared the incidence of thrombosis in abdominal surgery and laparoscopic surgery,the difference was not statistically significant.Compared with the control group,the diameter of femoral vein and popliteal vein in thrombosis group was widened,and the flow velocity of femoral vein and popliteal vein in thrombosis group was reduced.They were significantly different（P0.05）.Compared with the D-dimer testing,Color Doppler ultrasound had high sensitivity and specificity,and it could be an effective way to check deep vein thrombosis. Conclusion Color Doppler ultrasound can accurately assess postoperative gynecologic oncology patients with deep vein thrombosis,and it has high clinical value.
